This is a highly responsible professional nursing position under supervision of the Sr. Community Health Nursing Supervisor. Duties include but are not limited to: provides community clinical nursing services by assessing client’s health status and unmet needs, participates in public health clinics such as immunization, family planning, std, primary care, child and adult physicals; and performs related duties as required. Interacts with a variety of health care providers and community organizations. Works Special Needs Shelter in Hamilton and/or Columbia County as needed. Participates in On-Call duties. Duties involve the full scope of community health nursing activities.


Provides optimum nursing skills/function in all health department clinics; performs interviews, assessments, lab tests, immunizations, administers medications, provides education and counseling, completes and tracks referrals, completes all necessary documentation. Records client’s medical information and vital signs, updates medical history as needed. Issues medications as indicated using nursing issuance protocols. Assists the providers with physical exams, referrals and informs providers of client’s physical status and condition; evaluates laboratory results and assists providers in follow-up with abnormal lab results. Administers vaccines; supports family planning and completes family planning visits by protocol. Facilitates client medication refill requests by reviewing client record, orders medications prior to physician evaluation of refill request. Provides hematology services for clients and community partners.


Responsible for Immunization program management to include but is not limited to ordering VFC, Adult, Program 17 and Hepatitis 09 vaccines; Submits monthly reports to FL. Shots and VFC Program Office; Monitors county compliance rates for AFIX rates; responsible for MOGE of clients in VFC and documents attempts to contact. Participates in Immunization conference calls, Webinars and provides guidance and training updates to staff or new employees when necessary. Responsible for employee health immunization program. Provides employees vaccines and monitors annual TB assessments of employees.


Back up for EPI and uses Merlin, FDENS to report communicable diseases. Participates in Bi-weekly in the absence of EPI nurse and multi-county regional Epidemiologist. Participates in Epidemiology outbreak investigations and exercises and assists with animal exposures and Rabies PEP.


Provides community outreach education and activities when scheduled. Assists in health fair activities, provides education and health screenings and provides documentation of such activities, consults with and/or provides technical assistance to public and private entities concerning health care.


Responsible for securing the designated information sets for purpose of protecting confidentiality, data integrity and appropriate access for all health, both confidential and public record, which is stored in hard copy and electronic forms.
