Your Specific Responsibilities:

The incumbent will empower persons living with HIV and their families by providing compassionate care, support, and assistance in making informed decisions about their health and well-being.   Develop client-centered goals based on individual strengths, identified needs, and resources with the overall objective of developing skills needed to reinforce treatment adherence and positive health outcomes. Serve individuals with multiple complex psychosocial and health related needs.  Coordinate care and link clients to support systems that include, but are not limited to, medical, mental health, substance abuse, intimate partner violence, dental, transportation, housing, and financial services.   Support individuals in identifying their strengths and potential barriers to accessing medical care and support services.

Case Management

  • Work with Clinic staff and front desk administration to conduct brief intake and enrollment screening to determine suitability and readiness for services.
  • Financial screening for RW Part C eligibility using CAREWARE data. (Process needs to be created)
  • Maintain share of cost payment files. (Process needs to be created)
  • Provide timely coordination and linkage of the client to appropriate support services and activities required to assist the client in achieving the goals and objectives in their care plan.
  • Engage individuals in education related to HIV, health, treatment adherence, support services, and navigating the medical care system.
  • Monitor medical appointments and medication adherence based on doctor recommendations assuring medical appointments scheduled no less than every six months.
  • Track viral suppression rates, CD4 counts, new and returning clients.
  • Facilitate monthly Provider Meetings between clinic staff and RW Part B Lead Agency.
  • Serves as a liaison between RW part B lead agency (AH) and the clinic.  Requests come from Director of Client Services, Intensive Case Managers (ICM) and MCM’s. 
  • Download notice of eligibility and insurance to HMS from CAREWARE.
  • Making appointments for clients when appropriate.
  • Client advocacy throughout the continuum of care.
  • Ensure right to privacy and confidentiality throughout the continuum of care.
  • Prepare for discharge/termination of services.
  • Document case closure through a written closing summary.

 

ADAP Program Backup for Ryan White Part B clients in Monroe County

  • Screening, updating, enrolling clients in ADAP program.
  • Assist with annual Marketplace enrollment.
  • Prescreen for Medicaid/Health insurance.
  • Assist ADAP clients with ordering medication through approved specialty pharmacies.
  • Data entry/upload information in the ADAP Provide System.
  • Foster communication among internal and external program elements to coordinate entry to care, linkage, ADAP coordination, medical case management, STI’s, other local AIDS organizations (RW Part B lead agency).

 

HIV Program Administration

 

Other duties as assigned:

  • Assists as needed in all Florida Department of Health - Monroe County clinical settings based on agency need

 

  • Emergency Duty: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. 
  • Confidentiality: Incumbent may have access to records containing Social Security numbers in the performance of their job duties.

  • Knowledge of and experience with HIV/AIDS, STD’s and/or Hepatitis and current related issues
  • Knowledge of and experience with Counseling, Testing and Linkage Services
  • Excellent verbal and written communication skills

  • Ability to develop program manuals, policies, procedures, standards and rules.
