MINIMUM QUALIFICATIONS:
- 4 years of direct experience within a health care setting as a social worker, or a bachelor’s degree or any combination of degree and experience (2 year degree and 2 years’ experience) may substitute for the required experience
- Must have excellent communication and interpersonal skills
- Must be fluent in the English language: reading, writing, speaking
- Must be willing and able to work weekends and holidays if needed
- Must have basic computer skills in Microsoft Office Suite (Excel, Outlook, and Word) and electronic medical records
- Must be willing and able to become a Certified Addiction Professional Certification within one year of employment
- Successful completion of the employment screening process to include, but not limited to, drug test and national background investigation

PREFERRED QUALIFICATIONS:
- Two or more years of experience in a health care setting
- One year of experience in a health care setting as a social worker
- Experience working in a nursing home or long-term care/120-bed skilled nursing facility
POSITION DESCRIPTION:
The incumbent works under the supervision of the Social Work Program Manager and is responsible to provide social services to meet the individual needs of residents. Will identify, plan, and implement a program for psychosocial, mental, and emotional needs of the resident while providing, developing, and/or aiding in the access of services to meet those needs.
- Interviews clients, reviews records, and confers with other professionals to evaluate mental and/or physical condition of client or patient
- Works with residents, families, significant others, and staff to provide support for taking a more active role in self-advocacy to improve the quality of life/care for residents
- Enable residents to maximize their individuality, well-being, quality of life, independence, and dignity
- Monitors, evaluates, and records client progress according to goals described in treatment and care plan
- Develops, maintains, and utilizes a listing of current community resources that are useful to residents and their families
- Maintains resident related databases and clinical records as requested
- Attends ICP meetings, backs up Social Work Services Program Manager in their absence, manages grievance logs, and backs up Admissions Nurse to include screening, touring, and signing applicants
- Meets all governmental requirements for social service documentation
- Coordinates with the VA Substance Abuse Treatment Team in providing care to residents and develops interventions in the Domiciliary
- Coordinate the Domiciliary program to assist residents in applying for all eligible revenue sources, including Medicare and Medicaid
- Provide orientation training to new admissions.
- Perform other related duties and projects as assigned
