OPS HUMAN SERVICES COUNSELOR III

Hourly Rate: $18.23

Paid Bi-weekly.

MINIMUM QUALIFICATIONS:

• Must have excellent communication and interpersonal skills
• Must be fluent in the English language: reading, writing, speaking
• Must be proficient in the use of Microsoft Office Excel, Outlook and Word
• Must be willing and able to work weekends and holidays if needed
• Successful completion of the employment screening process to include, but not limited to, drug test and national background investigation

PREFERRED QUALIFICATIONS:

• Formal Education, bachelor’s degree. Direct experience may substitute for the degree requirement on a year for year basis.
• Experience with Medicaid eligibility in a long-term care facility
• Experience working in FLMMIS

POSITION DESCRIPTION:

• Works under the supervision of the Business Manager
• Complete intake forms on all residents upon admissions
• Provide knowledge of Medicaid benefits to residents
• Coordinate and complete assessment appointments
• Track progress and renewals for new residents and existing residents
• Complete and submit Medicaid applications and renewal applications
• Prepare monthly reports for billing
• Complete documentation gathering, faxing, scanning and copying
• Perform other duties as assigned.
