This is a professional position assigned to the Columbia County Health Department and reports to the Social Work Services Program Manager. The responsibilities include but are not limited to case management of Healthy Start clients. Healthy Start services will be provided to pregnant women, postpartum women, interconceptional women, and children (newborn to 3 y.o.) with the goals of improving birth outcomes and infant health.  The RN will provide an initial assessment to all clients to assess risk, educational level, and the need for services. Assists clients in obtaining community resources and services through referrals to community agencies.  Provides education and support to clients on parenting, childbirth, breastfeeding, family planning, nutrition, and smoking cessation.  Conducts face to face visits with clients in the home, doctor’s office, health department, or wherever convenient for client. Attends meetings, trainings, conference calls, and webinars to stay up to date with ways to better serve clients and to improve county rates for teen pregnancy, infant mortality, preconception & interconceptional health of women. Will coordinate the needs of Healthy Start clients in Columbia and/or Hamilton County as assigned to improve pregnancy outcomes for at risk clients. Will remain in compliance with the Healthy Start Standards and Guidelines. Duties include care coordination, chart compliance, and following the Healthy Start Pathways. Will secure and protect all information to protect confidentiality, data integrity, and appropriate access for all information confidential and public record being stored in hard copy and electronic form. Will participate in in annual program review with the Healthy Start QA/QI team and make changes as advised. Will participate in community outreaches as assigned by the supervisor, the Department, and the Healthy Start Coalition to maintain a high visibility in the community while fostering partnerships with community agencies. Participates in trainings to become certified and maintain certification in services provided by Healthy Start. Needs will be assessed, and referrals provided to other agencies in collaboration with Healthy Start. Help Clients obtain Medicaid services and services to OB care.  Will attend trainings and meetings as assigned by the Healthy Start Coalition, the Department, and supervisor. Basic computer literacy, valid car insurance, valid driver’s license, accessibility to reliable motor vehicle, ability to travel extensively, CPR certification, associate degree and RN license are required. Clients will now be seen in all the following delivery options: in-person, virtual, in-home visit, community event meetings, and add night and weekend hours to accommodate the clients working schedule. Position may also be asked to provide relief for clinical programs, as needed, to include STI, Family Planning, Immunization, and Ryan White clinics.

 

Responsible for duties to include but not limited to performing care coordination services, maintain compliance of charts, and following the Healthy Start Pathways. Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity, and appropriate access for all information both confidential and public record which is stored in hard copy and electronic form. Participates in annual program review with the Healthy Start Coalition QA/QI team and makes changes as advised. Will participated in community outreaches as assigned for the Healthy Start program and the Health Department. Will provide to the client’s satisfaction surveys and participate in quality assurance programs as related to the Healthy Start Standards and guidelines. Provides case management services and Healthy Start screening and assessments per the prenatal, ICC, and infant pathways. RN will maintain high visibility in the community while fostering community partnerships. Clients will now be seen in all the following delivery options: in-person, virtual, in-home visit, community event meetings, and add night and weekend hours to accommodate the clients working schedule.

 

Participate in trainings to get certified and maintain certification in services provided by Healthy Start. Assess the educational needs of clients who are at risk for poor pregnancy outcomes and provide the educational services or refer them out to community services if it cannot be completed by the RN. Help clients obtain Medicaid services as needed.

 

Designs and develops partnerships with clients, community partners, and healthcare providers to meet the needs of the Healthy Start clients. This includes but is not limited to the following:

    • Provide prenatal skill building involvement and support to Healthy Start clients.
    • Provide childbirth education with the Healthy Start approved curriculum.
    • Provide tobacco cessation education with the Healthy Start approved curriculum.
    • Provide parenting education with the Healthy Start approved curriculum.
    • Obtain and maintain certifications for all services required by the Healthy Start Coalition and the Health Department.

 

Performs other duties as assigned by the supervisor and/or Healthy Start Program which includes, but is not limited to weekly EAR submission and certification, timesheet submission, attend staff meeting, Healthy Start meetings, and community meetings as assigned. Completion of mandatory trainings in TRAIN, the Healthy Start LMS training data base, and any other trainings needed. Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity, and appropriate access, for all information, both confidential and public record, which is stored in hard copy or electronic formats. Performs other duties as assigned.
