MINIMUM QUALIFICATIONS:

  • Successful completion of the background screening process including, but not limited to a drug test and finger printing.
  • Better than basic computer skills.
  • Must be fluent in English: Reading, writing, speaking and understanding.
  • Must possess good customer service and interpersonal skills.
  • Must be willing to work weekends and holidays as scheduled.
  • Must be able to work all shifts (7-3, 3-11 and 11-7)

 

POSITION EXPECTATIONS:

  • The incumbent Health Support Aide (Personal Care Attendant) must participate in a training program and competency evaluation.
  • The incumbent will ensure completion of residents Activities of Daily Living ‎ (ADL) and other daily assignments during ‎the shift, while identifying and resolving resident needs.  This includes maintaining resident daily hygiene, nourishment and ‎appointments.‎
  • At completion of training, must successfully pass the CNA certification test within 4 months. 

 

The incumbent will:

  • Provide necessary assistance to residents when giving the following ADL nursing care – oral ‎hygiene; bathing; dressing and grooming; hair and nail care; toileting; assistance with meals, nourishments and ‎transportation to meal service areas, and mobility.* 
  • Perform simple nursing measures within their scope of permissible work duties.*
  • Routinely observe, identify and report resident concerns/needs to ‎supervisor.
  • Escort to resident appointments when required.
  • Complete assignments timely, completely and ‎accurately. Accept assigned duties in a cooperative manner.‎
  • Document in the nursing assistant notes the care and treatment provided to the resident.*
  • Complete required documentation. ‎
  • Consistently work   cooperatively with resident, resident representatives, facility staff, physicians, consultants and ‎ancillary service providers. Maintain Confidentiality of resident.‎
  • Follow established performance standards and perform duties according to nursing service policies and procedures. ‎Review care plans and perform nursing care as outlined. Participate in the development of an individualized plan of ‎care of assigned residents. ‎
  • Performs all other related duties in an effective, timely, professional manner as assigned.
  • Attend all in-service ‎programs classes as assigned
