Specific Responsibilities:

This is a Senior Community Health Nurse assigned to the Lantana/LW Health Center. Working in the Maternity/Family Planning/Sexual Health & GYN department; This position will be cross trained to work in all clinics as needed. Work is performed under the supervision of the Community Health Nursing Consultant. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. Adhere to established safety procedures according Florida Department of Health-PBC rules regulations, policies and procedures and practices in the work area.

 

This position is designated as a supervisor position and the position is required to maintain confidential information in accordance with the Florida Department of Health (FDOH and Palm Beach County Information Security Policy, Protocols and Procedures.   Confidential Data Sets: Clinical Information, Surveillance Tools & Reporting System (STARS), Epidemiology, Emdeon, Florida Shots, WIC, Employee Health Records (EHR). This position may review, update and release information from the Health Management System (HMS) and the Florida Shots Immunization System.

 

Responsible to perform assessments, care and/or treatments primarily in the Maternity/Family Planning/Sexual Health & GYN Clinics. Assist as needed with adult Immunizations. Advises patients and families on proper health care, interprets medication directions and factors related to patients’ condition for maintenance of good health status. Provide nursing care as well as education to the patients. Participates as a team member in obtaining and recording patient’s health, social histories and assessing patient needs.

 

Assist Providers as needed for the total care of all patients. Assist with discharge plans for clinic patients, receive and review lab, recording correct results in patient medical record. Performs immunization assessment and gives vaccines as needed. Performs patient home visits. Provide triage by phone and for walk-in patients without appointments. Schedules referral appointments, and return follow up appointments for patients. Assist in orientating new nurses and works as a preceptor for new nurses in the Maternity/Family Planning/STD & GYN department as directed by supervisor.

 

Reviews lab consultation reports and make appropriate disposition. Maintain a spread sheet for needed reports on abnormal PAP reports, and Immunization results. Assist with data collection and documentation as required.

 

Lead weekly teen clinic groups with topics such as: Family Planning, STD and nutrition. Visit school’s faith-based organizations and other community agencies on a monthly basis for the purpose of teen recruitment. Coordinate guest speakers on topics relevant to the teen health.

 

Participates in scheduled team meetings for discussion and management of patient care. Participates in planned in-service education sessions and Quality Assurance program.

 

Perform other duties as assigned.

Licensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite):

  • This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes and in accordance with Rule 64B9, Florida Administrative Code.

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. 
  • Valid Driver’s license.
  • This position may travel to another Health Center to provide services at the discretion of Administration.
