This Registered Nurse Specialist position is responsible for Healthy Families Care Coordination through the following duties and responsibilities:
Nursing Consultation/ Professional Analysis
a) Accepts referrals for all pregnant women/new babies and conducts a comprehensive nursing assessment;
b) Provides nursing assessments on pregnant women who develop medical problems or do not have routine prenatal care;
c) Tracks the weight and progress of all Healthy Families target children and siblings who have been; diagnosed as “failure to thrive”; identify as having a low birth weight; or assessed on the Ages and Stages Questionnaire as being developmentally delayed “working with the medical provider to ensure families follow through with appropriate community resources”; the nurse will provide consultation and education to staff regarding specified children with developmental delays.
d) Provides home visits on Healthy Families participants referred for any of the following reasons; low birth weight, prematurity, feeding problems, communicable diseases, illness, growth/development problems, poor bonding, family health issues, medical neglect and other health issues.
e) Provides education to interconceptional women, promoting healthy behaviors including SBE-self breast examination.
f) Provides tobacco risks education and counseling and smoking cessation.
g) Provides discharge/transition planning for program participants
Nursing/Care Coordination:
a) Educates families in basic health principals related to disease control, maternal and child health, family planning and safety, with emphasis on prevention.
b) Provides immunizations and well child care for families having trouble accessing the services.
c) Provides family planning services, as needed, to encourage baby spacing.
d) Follows all guidelines under nursing protocol.
e) Participates in DOH campaigns like Back to School Immunizations Rush.
Documentation and Other Indirect Services:
a) Documents all observations and actions in the client electronic medical record.
b) Completes necessary forms and adhere to state, program and Title X requirements.
c) Enters all documentation, including EARs and CSRs, within required timeframes.
d) Consultation with care coordinators about client’s medical services.
e) Presentations to staff on medical and health topics.
Quality Assurance:
a) Maintains valid Florida Nursing License.
b) Completes all mandatory trainings for program, division, and agency and for licensure.
c) Attends annual staff and nursing in-services and Nursing Action Team meetings when scheduled by the agency.
d) Abides by all policies and procedures of the Florida Department of Health in Pinellas County and those established by the Healthy Families Pinellas program.
e) Meets with supervisor on a regular basis and keeps supervisor informed of pertinent client and staff issues.
f) Participates in staff meetings, parent groups and graduation activities to cultivate staff and participant relationships
Performs all other duties as assigned.
Knowledge, Skill, and Abilities
Knowledge, skills and abilities, including utilization of equipment, required for the position:
Knowledge of nursing principles, practices, techniques and legal requirements.
Knowledge of community organizations and resources.
Ability to plan, organize, and coordinate work assignments.
Ability to communicate effectively verbally and in writing.
Knowledge of basic computer software and ability to enter data in computer.
Minimum Qualifications
· Valid nursing license per Nurse Practice Act Chapter 464, Florida Statutes 464.008, 464.009
· 1 year of experience working with maternal child health or pediatrics
· Valid Driver's license
· Access daily to reliable transportation
· At least 3 years experience working in public health/community health
Preferred Qualifications
· Home-Visiting experience
· Experience in working with pregnant women, young children and/or families
· Experience in providing family planning
· Bi-lingual preferred, English and Spanish speaking
