Your Specific Responsibilities:
This position provides, comprehensive public health nursing services and comprehensive case management in the Tuberculosis (TB), and Communicable Disease program, utilizing the public health nursing process. This individual will provide public health level assessment of health status and health care needs of individuals, families, and/ or groups of individuals in a variety of settings, including but not limited to clinics, schools, hospitals, homes and other public/community facilities. They will be responsible for direct patient care along with in-depth teaching and physical assessment that is directed toward the prevention, promotion and maintenance of optimal health. Work is performed under the direction and guidance of the Senior Community Health Nursing Supervisor.
Need to know position with access to medical records and social security numbers. Has computer access to Health Management System HMS and Florida Shots immunization program. Has access to secured areas and prescription medications. Adheres to Florida Department of Health in Citrus County Confidentiality, Security policy and procedures and attends annual confidentiality and security trainings and updates. Successful completion of a urine drug screen is a condition of employment. This position works primarily in the TB, and Communicable Disease, but may be assigned to any clinic location or program within the Florida Department of Health in Citrus.
Duties related to: TB, Communicable diseases, and Immunizations including but not limited to:
- Case management of TB clients.
- Screening and testing for TB.
- Identifying and testing contacts of TB Disease cases.
- Interviews cases by phone or when necessary, conducts field visits for interviews.
- Direct observed Therapy activities
- Conducts notifiable disease surveillance, investigation, and reporting.
- Educates client, family members and contacts regarding reportable diseases, disease process and transmission and control measures.
- Utilizes language line.
- Patient assessment
- Nursing interviews
- Planning patient care
- Implementation of provider orders
- Evaluation of response to treatment
- Administration of medications and vaccines
- Education and counselling
- Serve as patient Advocate
- Documentation of all information in Electronic Medical Record (HMS)
- Utilization of all applicable systems including but not limited to; Merlin, Careware, INFINITT, EDN, TB portal of HMS, ADAP Provide, Essence, STARS
- Follow up and case management of diagnostic studies.
- Referral management and tracking
- RN protocol nurse issuance
- Specimen collection and processing
- Supervision of professional and paraprofessional staff.
- Responds to patient questions and concern in person and via phone.
General duties/responsibilities:

- Enters time in People First and EARs according to policy
- Completes all required trainings within required time frames.
- Teaching educational programs utilizing area of expertise.

Required Knowledge, Skills, and Abilities:
- Knowledge of epidemiologic principles and practices related to public health surveillance, case investigation, outbreak detection and disease control.
- Knowledge of nursing principles, practices and techniques.
- Knowledge of health promotion and disease prevention.
- Knowledge of Immunization practices and administration of vaccines to children and adults.
- Knowledge of case management skills and investigative and interview techniques.
- Knowledge of Infection Control practices

Qualifications:
Minimum - Florida Statute Reference: Chapter 464 eligible to practice nursing in accordance with FL Admin. Code 210-8.22 or 8.27
Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity and appropriate access for all information, both confidential and public record, which is stored in hard copy and electronic forms.
Licensed to practice Registered Nurse in the state of Florida.
Must maintain a valid Florida driver’s license.
Must be willing to undergo Fit Testing.
Preferred – TB, and Communicable Disease experience.
