Your Specific Responsibilities:
This position performs a variety of duties for the Community Health Section in Brevard County. Duties include, but are not limited to, strong, effective time management skills that encompass field work in all county communities. Patient interviews and re-interviews, community education/outreach; case management, record maintenance in multiple electronic medical record programs. Assists clinics with locating patients. Maintains rapport with community partners as well as providing guidance with current CDC guidelines for the treatment of communicable diseases.
Required Knowledge, Skills, and Abilities:
Maintains knowledge of current CDC STD management , treatment and investigation.
Provides timely investigation and case management according to the diagnosis and needs of the patient under the direction of the physician in charge. Services include teaching, counseling, and coordination of patient care to ensure completion of therapy. Maintains/updates databases and records along with completing all programmatic reporting requirements.
Assists medical clinics; Implements recommendations and follow-up on physician’s orders. Provides community outreach to various partners by working with other health agencies, civic groups and service clubs.
Participate in epidemiological investigations, screening programs, and educational activities for the promotion of community partnerships as well as community education and awareness. Maintains appropriate confidentiality.
Performs other related duties as assigned. Communicates with courtesy, respect and understanding in dealing with clients, the public and staff.
Coordinates and conducts timely management of field visit travel, field records assigned, patient interviews, re-interviews. Verifies appropriate treatment of patients infected with any communicable disease for the purpose of: (1) educating/counseling patients on their infections, coordinating appropriate treatment (2) developing plans to locate and refer sex partners, or others at risk, for exposure/contact to a communicable disease to the DOH or appropriate medical facility.
Position requires daily in-office or field work. Ability to stand for at least an hour. Ability to lift 15 pounds.
Qualifications:
Minimum: Four (4) years or more experience as a disease intervention specialist in healthcare setting, Current HIV 501 Certificate (F>S. 38.004). Valid Florida driver’s license. Ability to travel to conduct field services. Ability to use multiple electronic health records or computer applications.
Preferred: Bachelor’s Degree, Ability to speak and understand Spanish. Previous experience as a Disease Intervention Specialist. Ability to provide phlebotomy services.
