Your Specific Responsibilities:
The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. This position will assist PrEP clients with services such as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, management of clinical costs, appointment reminders, education on the benefits of PrEP and ongoing maintenance. This position requires firsthand knowledge of public health systems, as well as professional and interpersonal skills to interact with health care clients, health care professionals and others who impact HIV/STD Prevention and Control, public health awareness and other CHD activities. Duties will also require knowledge of varied social, economic, cultural and lifestyle characteristics, and skill in communicating with, and eliciting information from patients and target groups with these diverse backgrounds. Minimal guidance and instruction on work methods, procedures and techniques are provided with a review of activities at completion for technical accuracy, timeliness and adherence to instructions and guidelines.
Work with STD clinical staff and others to identify potential PrEP clients and those in need of PrEP navigation services.
Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.
Complete screenings to identify readiness for PrEP and other barriers to care. Work with client to identify possible solutions.
Assess benefits and insurance, and if needed, provide assistance to access payor programs.
Provide PrEP disease prevention and PrEP adherence education and counseling.
Create procedure for injectable PrEP. Daily follow up with patients to verify appointments, update database, verify delivery of medication. Monitor Q-flow with the goal of reducing patient cycle time, receiving E-Faxes, Patient scheduling for initial and follow up appointments, schedule overbooks to accommodate patient needs.
Review patient appeals and follows up with pharmacy provider to ensure information requested is provided in a timely manner
Facilitate Telehealth appointments by contacting patients to schedule, assisting with lab orders, complete financials, and insurance verification. Contact patients before appointment to confirm and ensure labs are entered
Navigate patients through Gilead Patient Assistance Program to ensure patient approval.
Assist in documenting program activities and reports outcomes in Microsoft Dynamics Patient Reporting System.
Effectively collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
Provide ongoing follow up to ensure PrEP adherence.
Maintain appropriate patient files and timely documentation of services utilizing applicable databases and software.
Performs related duties as assigned.
Required Knowledge, Skills, and Abilities:

Qualifications:
Minimum-
Valid driver’s license and access to an automobile to perform work related duties, experience performing case management activities.
Preferred -
The incumbent in this position is preferred to be fluent in English/Spanish or English/Creole.
