This is a Disease Intervention Specialist providing Linkage to Care services. This position requires firsthand knowledge of public health systems, as well as professional and interpersonal skills to interact with health care clients, health care professionals and others who impact STD Prevention and Control, public health awareness and other CHD activities. Duties will also require knowledge of varied social, economic, cultural and lifestyle characteristics, and a skill in communicating with, and eliciting information from patients and target groups with these diverse backgrounds. Minimal guidance and instruction on work methods, procedures and techniques are provided with a review of activities at completion for technical accuracy, timeliness and adherence to instructions and guidelines. The incumbent is required to link clients to medical treatment services and must be able to work collaboratively with clinical and other key staff that plays a role in providing Linkage to Care services to the clients of Miami Dade County. All activities performed must comply with federal, state, department and CHD information Security and Confidentiality statutes, rules, regulations, policies and procedures.


Locates and links clients infected with HIV to medical treatment services as well as any other services needed. This will be done through collaboration with HIV Surveillance, community partners, as well as the HIV/STD Prevention and Control Program. Travels to residences, clinics and other locations as needed. Consults with private physicians; private and public laboratory personnel, and other health care providers to: (1) stimulate case reporting; (2) obtain/provide information on treated cases or diagnostic determination (3) document findings via methods established the STD/ HIV Prevention and Control Program within established timeframes.


Performs case management activities to include preparing and maintaining timely and accurate records and reports of the linkage to care process for program evaluation purposes and other records and reports as required and submits to the Monitoring and Evaluation Coordinator.  Documents field activities as required. Utilizes PRISM to input cases in an accurate and timely manner.


Provides PCRS in accordance with current STD HIV/AIDS Program policy to a wide variety of individuals infected with or at-risk of an STD, hepatitis, HIV/AIDS or other bacterial, biological and/or viral agent to elicit partner and suspect information. Utilizes information for epidemiological analysis to identify the source and spread cases related to the known infection.


Assistance in additional office related activities and duties as assigned. Participates in any committees, events, special programs or initiatives celebrating National Days of Observance or other activities organized, sponsored or supported by the Office of HIV/AIDS, MDCHD, Florida DOH or as determined, required or needed by the program and/or the county health department administration.


This position requires the candidate to have experience working in an environment with culturally diverse clients, heavy client volume and challenging assignments.  Be willing to perform home visits in high-risk areas of the county.  Be willing to perform needle sticks to draw blood from an HIV/STD/TB client after being properly trained.  Have taken and satisfactorily completed the AIDS 500/501 class.  Be willing to travel during the workday. Are you willing to travel within the Miami-Dade County geographical area.  Have a valid driver’s license and access to an automobile to perform work related duties.  Have experience linking clients to medical treatment services.  Have at least two years of professional experience working in a disease control program, social services program, or community based organization. 



Preference will be given to those candidates who have a Bachelor's degree.  Who are bilingual in English/Spanish.
