Your Specific Responsibilities:

This is a Disease Intervention Specialist providing Linkage to Care services. This position requires firsthand knowledge of public health systems, as well as professional and interpersonal skills to interact with health care clients, health care professionals and others who impact STD Prevention and Control, public health awareness and other CHD activities. Duties will also require knowledge of varied social, economic, cultural and lifestyle characteristics, ability to work in a fast-paced environment and a skill in communicating with and eliciting information from patients and target groups with these diverse backgrounds. Minimal guidance and instruction on work methods, procedures and techniques are provided with a review of activities at completion for technical accuracy, timeliness and adherence to instructions and guidelines. The incumbent is required to link clients to medical treatment services and must be able to work collaboratively with clinical and other key staff that plays a role in providing Linkage to Care services to the clients of Miami Dade County. All activities performed must comply with federal, state, department and CHD information Security and Confidentiality statutes, rules, regulations, policies and procedures. On-medical case management duties including but not limited to: Initial comprehensive assessment of service needs.  Timely and coordinated access to medically appropriate levels of health, prevention and support services and continuity of care.  Eligibility Determination for Ryan White Part B services.

 

Locates and links clients diagnosed with HIV to medical treatment services as well as any other services needed. This will be done through collaboration with HIV Surveillance, Test and Treat Rapid Access (TTRA), community partners, as well as the HIV/STD Prevention and Control Program. Complete field visits to client’s residences, shelters, clinics, hospitals and other locations as needed. Consults with private physicians; private and public laboratory personnel, and other health care providers to: (1) stimulate case reporting; (2) obtain/provide information on treated cases or diagnostic determination (3) document findings via methods established the STD/ HIV Prevention and Control Program within established timeframes (4) refer clients to partner agencies to receive medical treatment (5) transport and or meet TTRA clients to initial medical appointment to ensure clients are linked to medical care.

 

Performs case management activities to include preparing and maintaining timely and accurate records and reports of the linkage to care process for program evaluation purposes and other records and reports as required and submits to the Monitoring and Evaluation Coordinator. Complete Partner Service Interviews with newly diagnosed HIV patients through the FOCUS Project.  Utilizes STARS to input partner services narrative in an accurate and timely manner. Utilizes HIV/AIDS Linkage Module to document all findings such as field activities, phone attempts, mail attempts, case notes, labs, verification of care, etc.

 

Provides PCRS in accordance with current STD HIV/AIDS Program policy to a wide variety of individuals infected with or at-risk of STD, hepatitis, HIV/AIDS or other bacterial, biological and/or viral agent to elicit partner and suspect information. Utilizes information for epidemiological analysis to identify the source and spread cases related to the known infection.

 

Assistance in additional office-related activities and duties as assigned. Participates in any committees, events, special programs or initiatives celebrating National Days of Observance or other activities organized, sponsored or supported by the Office of HIV/AIDS, MDCHD, Florida DOH or as determined, required or needed by the program and/or the county health department administration.

  • Ability to maintain strict confidentiality.

Qualifications:

Minimum –

Valid driver’s license, access to reliable vehicle to perform job related duties, experience working in a disease control program, social services program, or community-based organization. 

Preference - 

Incumbent in this position is preferred to be bilingual in English and Spanish.
