$41,000.00 - $44,290.22 Annually            $1,576.92 - $1,703.47 Bi-weekly

 

Position Summary

This is a responsible position that requires independent judgement, balancing multiple priorities, and an ability to present and communicate with families, multiple agencies, and various levels of management. The positions’ primary responsibility is to coordinate and authorize residential placements in APD licensed group and foster homes.

 

The Work You Will Do 

  • Schedule and lead all calls related to residential planning including transition calls for individuals moving from one between community settings, chair the weekly residential planning meeting with the regional team and representatives from State Office, develop meeting agenda and send to team members one day before the call, and responding to questions concerning referrals, participate in CBC calls as the Agency representative to provide updates on referral status.
  • Serve as the Agency point of contact in matters related to residential planning. Contact crisis stabilization units (CSUs) and hospitals when a consumer is slated for discharge, provide training on the residential planning process at scheduled provider waiver support coordinator meetings as requested, speak directly with families WSCs and providers to discuss pending placements.
  • Conduct follow up activities related to residential plans for: clients who have received 30-day notices from providers, group home closures, emergency/respite requests. Follow up with waiver support coordinators (WSCs) who do not submit residential referral forms timely or correct and providers who are not following the planning process by submitting residential response form. As needed, request a review of the potential placement by the behavioral team to ensure health and safety of the clients can be maintained, review cost plans to ensure that the client’s current cost plan has sufficient dollars to fund the placement and alert the Regional waiver liaison if a Significant Additional Needs (SAN) is needed prior to placement being approved. Alert Regional Operations Manager (ROM) or designee if IFS funding will be needed prior to waiver enrollment of client.
  • Maintain and utilize the Agency residential tracker which reflects all individuals currently in need of placement, preferred location and type of setting needed in the Region.
  • Update and maintain Group Home and Residential Facilities Data (GRID) with confirmed moves as they occur to ensure the individual is properly associated with the correct living setting.
  • Conduct research on the current residential placement status of individuals who are brought to the attention of APD leadership, provider update and resolve any issues which may require travel to community locations.
  • Perform other related duties as directed including providing assistance with emergency/disaster preparedness activities.

 

Minimum Qualifications

  • High School Diploma or its equivalent.
  • Must have a valid Driver’s License or other efficient means of transportation.
  • Must have a minimum of two years of experience in working with people with disabilities, care coordination, or in the health and human services field.
  • Must have experience in service needs for individuals with developmental disabilities.
  • May require travel to community locations.

 

Knowledge, Skills, And Abilities

Knowledge of:

  • Microsoft Office applications including Word, Excel, SharePoint, and PowerPoint and the ability to use applications to complete daily responsibilities.
  • Proper grammar, punctuation, and spelling to write coherent and complete necessary documents required as part of your job duties.
  • Clinical language, APD services and resources available to clients.

Ability to:

  • Apply case management practices to the facilitation of resident transition.
  • Work in precise and stressful situations; to probe and obtain critical facts about residents.
  • Plan, organize and coordinate work assignments; ability to communicate effectively both orally and in writing.
  • Describe facts and events accurately.

As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. Retirement funds are also required to be in the Direct Deposit Program.

Background Screening Requirement
