This is a professional clinical nursing position for the Medical Foster Care (MFC) Program reporting to the Registered Nursing Supervisor. The individual in this position administers pediatric nursing care utilizing the nursing process in the assessment, education, implementation, and evaluation of the health care delivery of services and MFC plan of care (POC). This includes evaluating and advising on health maintenance, disease prevention, and management of medically complex care of a child in the home environment.

 

Your Specific Responsibilities:

 

Assessment

Completes pre-service assessment, interview, and written assessment of prospective MFC parents and their home environment, including a home visit for on-site observation review.

Assesses MFC patient referrals to determine suitable match of individual child/youth medically complex needs with available MFC parent provider with the appropriate clinical skills and home dynamics. Concurs reviews of appropriate matches with MFC staff including Medical Director for consideration of recommendation for admission.

Provides ongoing assessment and surveillance of the MFC parent and child dyad in the home environment, which includes implementation of prescribed treatments and interventions as outlined in the MFC POC. Interprets, and/or evaluates medical orders, and medical records to assess patient's condition, and needed updates to the MFC POC.

Safeguards the provision of necessary equipment, medications, supplies, and other necessary patient specific resources and supports.

Provides ongoing assessment information and discusses cases with MFC staff including Medical Director.

Completes periodic verbal and written assessments of MFC parent providers’ care of child(ren) in home and adherence to program standards.

Reviews data, assessments, and documentation verifying accuracy, quality, and completion of goals or outcomes.

Monitors the quality of care or standards of practice to ensure compliance with program, state and federal requirements.

Develops and monitors technical assistance or corrective action plans of MFC parent services for quality improvement and adherence to program standards and policies. Provides continuous assessment to assure correction and sustainability of performance with desired outcomes.

Responds to special directions and instructions during emergency operations inclusive of providing assessment support pre, during and post emergencies.

 

Communication

Provides quality verbal and written communication with MFC and community stakeholders to ensure understanding and engagement throughout process. Facilitates coordination in the scheduling of MFC staffing’s ensuring timely notification and engagement of necessary MFC stakeholders including, but not limited, to the MFC client’s Medicaid Managed Medical Assistance (MMA) plan and CBC representative.

Engages, advocates, and supports opportunities for current or future caregiver participation, as appropriate, and family-centered approaches in the MFC program.

Provides verbal and written nursing education and documentation of training and technical assistance to MFC and community stakeholders.

Provides consistent messaging that aligns with statutes, rules policies, procedures, and processes which affect decisions, determinations, and management of the program.

Responds to regional and statewide requests for information about MFC program coordination.

Acts as the MFC program liaison in communication with other community providers and stakeholders.

Serves as a liaison between the MFC provider and the MMA Plan.

Attends and participates in other meetings as assigned.

 

Education and Training

Provides pre-service MFC training for foster parents considering the MFC program.

Provides or assists in the provision of MFC program onboarding training. Provides or coordinates and oversees annual training requirements. Tracks program training requirements to ensure completion.

Provides or oversees child/youth specific training. Child/youth specific training includes comprehensive training with return understanding or demonstration of necessary knowledge and skills needed to care for medically complex needs of child/youth being admitted into the MFC parent home. Provides ongoing child/youth specific training based on evolving and changing needs of child/youth and MFC parent. Provides or oversees the provision of child/youth specific training with biological or adoptive parents in preparation for reunification discharge from the MFC program.

Provides training and technical assistance to MFC parent to ensure program requirements and MFC parent skill requirements are exceeded or met.

Provides community outreach and education to community stakeholders regarding the MFC program and requirements.

Provides community outreach and education to community stakeholders to identify potential families for MFC parent recruitment.

 

Implementation

Responsible for all day-to-day nursing aspects of the MFC Program.

Assists with and monitors all aspects of patient care in the implementation of the child’s MFC POC.

Assists with and monitors all aspects of child health care and supervision in the implementation of the child’s MFC POC.

Advises on health maintenance and disease prevention.

Responsible for nursing education or skill training based on individual child/youth diagnosis, symptom management, medication administration, clinical interventions, durable medical equipment needs, nutritional support, or other identified needs.

Develops or selects curriculum and prepares instructional information, and other materials used in teaching. Ensuring return knowledge or demonstration of education or training, documenting the resulting outcome.

Completes other nursing activities needed to ensure a safe admission and ongoing health supervision and care into the program and MFC provider home. Documents activities accordingly.

Provides ongoing nursing support of child and MFC parent according to program requirements, provides or facilitates case management.

Monitors the reporting and recording of symptoms or changes in patients’ conditions, providing nursing expertise and coordination.

Prepares and reviews child/youth specific needs with the MFC parent. Providing professional verbal and written communication aligned with program and agency objectives.

Completes nursing visits to home and community as indicated

Attends and participates in meetings related to care and service provision of child, and other meetings as assigned.

Provides community outreach and education to community stakeholders regarding the MFC program and requirements or to identify potential families for MFC parent recruitment.

Implements organization, program, and individual quality assurance standards in conjunction with management.

Participates in program quality assurance and improvement needs.

Implements activities identified in the Plan, Do, Study, Act (PDSA) cycle, to ensure that continuous quality improvement activities address suboptimal findings.

 

Quality Assurance

Maintain, review, and monitor MFC related data and Electronic Health Record entries.

Reviews data, assessments and documentation verifying accuracy and quality as they are being completed. In coordination with MFC stakeholders, such as the CBC or licensing agency and MMA plan, evaluates MFC parent

skills, competencies, documentation, timeliness, quality of services provided and other indicated data points.

Develops and monitors technical assistance plans for MFC parent for quality improvement. Provides continuous monitoring to assure correction and sustainability of performance with desired thresholds.

Implements organization, program, and individual quality assurance standards, in conjunction with management. Provides monitoring of program activities for quality assurance purposes. Participates in program quality assurance and improvement needs. Implements documented activities identified in the Plan, Do, Study, Act (PDSA) cycles.

Participates in central office training, evaluation, monitoring, and related follow-up activities to improve MFC processes, activities, and outcomes. Provides reports from data base systems as requested by management.
