THIS IS A CAREER SERVICE POSITION

The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children ages 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. REQUIREMENTS:

Must have a high school diploma or its equivalent.

 

Must have at least two years of experience working with individuals with developmental disabilities.

 

Must have experience working with Microsoft Office software such as Excel and Word.

DUTIES:

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service.  This includes, but not limited to, acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

 

This is a responsible professional position which involves providing support coordination to consumers with special needs, resulting from developmental disability.  An employee in this position is responsible for maintaining orderly central record to include the following:

 

Consults with individuals and families to identify resources as well as finding and gaining access to natural supports and community resources.  Advocates on behalf of the individuals and families. Coordinates and monitors the delivery of APD funded supports and services to ensure appropriate delivery of the service and the extent to which the service is meeting the individuals and family’s needs and expectations which may require travel to community settings including consumer homes, licensed group home facilities or other settings in the field.

 

Maintain accurate information on the iConnect System for all consumers following case closure protocols as required.

 

Ensures invoices are authorized and prepared within 5 working days of bills received or services approved, using ABC systems and/or other technology for payment and in compliance with the mandatory requirements in Section 215.422, Florida Statutes and Chapter 3A-20, Federal Administration Code.

 

Creates and updates the support plan on an annual basis. Completes an Annual Status Review and Annual Report in the I Connect System on an annual basis. Follows up with possible need of services requested through the Annual Status Report Form. Documents contacts and case management activities in central record/iConnect system.  Reviews central record to ensure compliance with operating procedures and standards.

 

Documenting ongoing progress notes in central record/iConnect system within 5 working days to explain coordination activities in support plan development, implementation, use of natural supports and justification of expenditures. Prepare and complete Crisis Tool request packets in a timely manner for individuals meeting the crisis criteria and any collateral information attached for those consumers who are in crisis at the moment. Completes required paperwork following procedures to transfer of Crisis approved consumers to the Home and Community-Based Services Waiver.

 

Directly assist with transitioning individuals from/to an ICF/IIDs; to include ICF/IID to Home Community Based, ICF/IID to another ICF/IID or Home Community Based to ICF/IID through the Choice Counseling Process within 90 calendar days.  Assist the Region’s Forensic Liaison in transitioning individuals from SOAR/Sunland Intermediate Care Facility to a Home Community Based Services. Ensure needed services are uninterrupted during transition process by issuing service authorizations through General Revenue.

 

Other duties and projects as assigned.

 

KNOWLEDGE, SKILLS, AND ABILITIES

  • Must have knowledge and at least two years professional work experience with Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences.
  • Ability to solve problems and make decisions.
  • Must have knowledge and proficient use of Microsoft applications (Excel, Word, Power Point Share Point) and other required computer software applications.
  • Must have excellent ability to communicate orally and in writing, (face to face, electronically and/or written) information and ideas clearly, concisely, and effectively. 
  • Must have the ability to establish and maintain effective working relationships with consumers, families, providers, and staff within and across regions.
  • Must have the ability to prioritize and schedule work efficiently, determine work priorities and ensure completion of work assignments.
  • Must have the ability to work independently.
  • Must have ability to multi-task and work under pressure.
  • Ability to travel for work purposes.

 

DIRECT DEPOSIT PROGRAM - As a condition of employment, a person appointed to a position in State government is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. BACKGROUND SCREENING REQUIREMENT

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
