Your Specific Responsibilities:

This is a highly responsible and independent, full time professional position responsible for community-based outreach and education efforts of the Community Health Division of the Florida Department of Health in Santa Rosa County. This position requires the ability to work independently and to work with both youth and adults. Public speaking, grant management, and program development skills are required. The person in this position must also have a working knowledge of computers and word processing skills.  The position requires some travel and weekend and evening hours. This is a tobacco-free position requiring a background check.

  Serves as Healthy Communities Grant Champion. Assists in developing plans and conducting health education activities with 5-2-1-0 and Healthiest Weight messages for community-based centers, childcare centers, and faith-based organizations. Collaborates with community partners to complete the Healthiest Weight Workplan and strategies. Documents and reports all Healthiest Weight activities to the state health office and Public Health Services Manager for Community Health. Participates in community meetings relevant to the healthiest weight activities.

Acts as the Community Outreach Liaison for the Santa Rosa County Health Department. Coordinates community-based health education events such as health fairs, marketing, and community presentations. This includes coordinating and participating in evening and weekend outreaches. Leads and/or participates in community groups and community activities and works cooperatively with other CHD divisions and agencies to meet the goals of Public Health. Tracks and documents all outreach activities and meetings. Speaks to community groups, civic organizations, businesses, and religious organizations. Conducts presentations at health fairs and community events as needed. Prepares and selects exhibits, posters and literature for dissemination through various communications media. 

Promotes and engages community partners to participate in Public Health Initiatives. Educates and collaborates with community partners to increase awareness of the Community Health Improvement Plan (CHIP) goals and objectives, behavioral health issues, child health and well-being, and healthy lifestyle activities.  Participates in trainings and meetings with community agencies to meet CHIP goals.  Works with Public Health Services Manager on Community Health Needs Assessment, Strategic Planning, Quality Improvement, and Accreditation processes. 

National Diabetes Prevention Facilitator:  Helps facilitate the National Diabetes Prevention Program (NDPP) for DOH Santa Rosa. Works with outside agencies and partners with health care systems and worksites to promote the NDPP. Works with Solera to increase the number of referrals to NDPP. Initiates the NDPP curriculum and classes for Inclusive DPP. Follows the guidelines and Medicare payment schedule for Medicare clients to initiate billing of NDPP. Co-facilitates classes at different NDPP sites. Collects data and prepares reports for submission to the CDC for continued site recognition. Assists with purchasing program incentives for NDPP.  

Works with the local Tobacco Prevention Specialist/Students Working Against Tobacco (SWAT) Coordinator to implement SWAT activities and trainings.  Promotes youth empowerment by increasing skills, knowledge of tobacco issues and prevention, and assists with advocacy training.  Plans and disseminates the Florida Youth Survey, yearly, to selected schools and classrooms as required by the State Tobacco Office. 

Attends trainings, mandatory quarterly, day-long health department meetings including, disaster preparedness training, as indicated, to enhance skills. Performs other duties as assigned or required related to disaster training and preparation.

Qualifications:

Incumbent must possess a valid Florida Driver’s license and have a motor vehicle to carry out these duties.
