Your Specific Responsibilities:

This is a highly responsible and independent, full time professional position responsible for community-based outreach and education efforts of the Community Health Division of the Florida Department of Health in Santa Rosa County (DOH-Santa Rosa). This position requires the ability to work independently and use critical thinking skills to work with both youth and adults. Public speaking, grant management, and program development skills are required. The person in this position must also have a working knowledge of computers and Microsoft office products.  The position requires some travel and weekend and evening hours. This is a tobacco-free position requiring a background check.

Serves as Healthy Communities Grant Champion. Assists in developing, conducting, and reporting health education activities and Healthy Community messaging for community-based centers, childcare centers, and faith-based organizations. Collaborates with community partners to complete the Healthy Communities workplan and strategies. Documents and reports all Healthy Communities activities to the state health office and Public Health Services Manager for Community Health. Participates in community meetings relevant to the healthy communities’ activities.

Promotes and engages community partners to participate in Public Health Initiatives. Educates and collaborates with community partners to increase awareness of the Community Health Improvement Plan (CHIP) goals and objectives. Participates in trainings and meetings with community agencies to meet CHIP goals. Works with the community health needs assessment and improvement plan coordinator to meet CHIP goals.

Acts as the Community Outreach Liaison for the Community Health and DOH-Santa Rosa. Coordinates participation in community-based health education events such as health fairs, lunch and learns, and community presentations. This includes coordinating and participating in evening and weekend outreaches periodically. Leads and/or participates in community groups and activities and works cooperatively with other CHD departments and agencies to meet the goals of the Department. Tracks and documents all outreach activities and meetings. Speaks to or coordinates speakers for community groups, civic organizations, businesses, and religious organizations. Prepares and selects exhibits, posters, promotional items, and literature for dissemination through various channels. Responsible for purchasing for the community health promotional items for outreaches.

National Diabetes Prevention Program (NDPP) Coordinator and Facilitator:  Leads the coordination of the national diabetes prevention program for DOH-Santa Rosa and participates as a facilitator of the National Diabetes Prevention Program for
DOH-Santa Rosa. Works with outside agencies and partners with health care systems and worksites to promote the NDPP. Works with Solera and local physician offices to increase the number of referrals to NDPP. Follows all guidelines and payment schedules to initiate billing of NDPP classes. Prepares for each class and co-facilitates classes. Collects and maintains client data and prepares reports for submission for continued site recognition. Assists with purchasing program incentives for NDPP. Provides educational presentations and information about the NDPP to the public and community stakeholders to increase class participation.

Lead facilitator for the DOH-Santa Rosa healthy babies meeting. Works with Healthy Start Santa Rosa Director to bring maternal and child health focused stakeholders together to discuss infant mortality and create workplans to address this topic. Manages email distribution list, sends out meetings items like the agenda. Becomes Certified passenger safety technician to assist in the proper installation and education of the DOH-Santa Rosa car seat program.

Attends trainings, mandatory quarterly, day-long health department meetings including, disaster preparedness training, as indicated, to enhance skills. Performs other duties as assigned or required related to disaster training and preparation.

 

Required Knowledge, Skills, and Abilities:

  • Knowledge of federal or state grant management.
  • Proficient skills in Microsoft Office, Excel, PowerPoint, Outlook, Publisher.
  • Ability to work independently and completes assignments in a timely manner.
  • Ability to prepare reports and professional presentations in a timely manner.
  • Skills to communicate effectively to multiple stakeholders in the county.
  • Ability to plan, implement, and evaluate community health programs.
  • Ability to work with a variety of community partners and facilitate collaboration.
