Your Specific Responsibilities:

Clinic Activity Supervision:  Responsible for the supervision of daily clinical activities of staff in the health department including planning and directing work and assures standardization of medical record documentation through ongoing QA activities.  Responsible for  the CHD clinics including General Clinic, Primary Care Clinic, OB Clinic, family planning clinic, immunizations, walk-in and triage, as well as programmatic activities ensuring that each program meets all state requirements.   Renders skilled nursing assessment to uncover health problems through observation, interviews and record analysis.  Administers preventive immunizations and treatments under medical direction, following written protocols and procedures of the CHD.  Assists physician/ARNP with examinations and treatments.  Obtains laboratory specimens and performs diagnostic tests following written guidelines and procedures.

Desk/Triage:  Provide assistance with patient referrals through other agencies and clinics, assist provider with patient follow-up as indicated ensuring complete and accurate documentation on all patients.  Scan patient records into HMS.  Answer and Triage incoming phone calls promptly redirecting calls as needed.

The Community Health Nurse in this position provides for all segments of the School Health program. Responsible for the direct oversight of staff including planning and directing their work. The primary work site will be at Jefferson Somerset Charter School. Responsibilities include, but are not limited to:

  • Provides direct oversight of Licensed Practical Nurses and Health Support Technicians,
  • Provides technical assistance and training to all staff
  • Promotes health activities for students and staff
  • Participates in faculty, parent/teacher organizations when needed
  • Plans for health class presentations and/or supervision of staff presentations
  • Monitors school clinic logs for accuracy on a weekly basis
  • Plans and assists in mandated health screenings and refers students in need of further medical evaluation
  • Provides Individualized healthcare plans and emergency care plans for students with diagnoses requiring care.
  • Supervises distribution of prescription and over-the-counter medications
  • Identifies students at risk for pregnancy, STDs, drug/alcohol abuse and/or suicide and refers students in need of treatment
  • Incumbent will participate in Quality Improvement Activities as directed.
  • Ensures student records are complete and include up to date immunization records and school health physicals.
  • Prepares annual School Health Report and bi-annual School Health Plan
  • Meets with School Health Staff on a frequent basis to ensure adherence to guidelines
  • Monitors high risk students for compliance and follow-up through medical referral
  • Develops and oversees budgets, including monitoring program spending and adjusting budgets.
  • This position is responsible for the preparation of purchasing and the approval of purchasing.
  • Serves as overall chairperson of the School Health Advisory Council ensuring at least bi-annual meetings
  • Participates in grant writing opportunities as needed
  • Promotes the School Health Program throughout the community
  • Performs other duties as assigned
     
    Required Knowledge, Skills, and Abilities:
  • Knowledge of software to enhance the productivity of the CHD Programs
  • Organization skills,
  • Communication skills with nursing staff, patients, families.
  • Abilities to work directly with patients, families, medical staff. Vaccinating adults and pediatric patients.
     

    Qualifications:
    Minimum
  • Florida RN License.
  • CPR/ Basic First Aid Certification
  • Minimum Computer skills
  • Valid Driver’s License
    Preferred
  • Infection control experience, pediatric nursing, COVID response experience. HMS/FL Shots/Merlin experience.
