Your Specific Responsibilities:
This is a professional level position that serves as a Health Education Program Consultant for the Florida Department of Health in Collier County (DOH-Collier) under the direction of the Community Health Promotion (CHP) Division Director. Coordinates and manages the development and implementation of strategies relating to emerging public health threats, community health assessments, injury prevention, mental health, chronic disease, and tobacco prevention. Responsible for conducting outreach and educational presentations to promote community health and provide health education awareness for injury prevention, chronic disease prevention, mental health, health of older adults and various other health initiatives. Duties include community engagement, building partnerships, conducting presentations, meeting facilitation, assessment, strategic planning, policy development and implementation, evaluation, and providing consultation and assistance to individuals and groups in one or more of these settings: community, school, work site, and health care. Collect and analyze data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. This position works closely with the Health Improvement Planning Program Manager to assist with the development and implementation of the Age-Friendly Action Plan, Community Health Assessments, Community Health Improvement Plan, and the Tobacco Prevention workplan. Responsible for implementing actions steps of the Healthy Communities workplan, documenting activities, and evaluating progress in achieving workplan goals and objectives.
Coordinates and manages the development and implementation of strategies relating to emerging public health threats, community health assessments, community health improvement plans, injury prevention, mental health, chronic disease, and tobacco prevention. Responsible for conducting outreach and educational presentations to promote community health and provide health education awareness for injury prevention, mental health, chronic disease prevention, health of older adults, and various other health initiatives. Coordinate lectures and health fairs for DOH-Collier. Duties include community engagement, building partnerships, conducting presentations, meeting facilitation, assessment, strategic planning, policy development and implementation, evaluation, and providing consultation and assistance to individuals and groups in one or more of these settings: community, school, work site, and health care. Assists with facilitating the development and coordination of the community health assessment, community health improvement plan, and annual progress reports through public health surveys, focus groups, meetings and interviews. Collect and analyze data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Facilitates Community Health Improvement Plan (CHIP) workgroups, creates agendas, and meeting minutes. Develop relationships and create partnerships with other public, private, and non-profit agencies. Represent DOH-Collier on specific health agency committees and boards, including the Behavioral Health lnteragency, Drowning Prevention committee, Bicycle and Pedestrian Advisory Committee, Tobacco Free Partnership, and more.
Responsible for the development and implementation of worksite wellness and awareness campaigns for year-round health education. Promote worksite wellness at the CHO by encouraging active living, healthy eating, smoking cessation, and stress management. Monitors, develops, and maintains the Healthy Communities workplan. Identify and pursue opportunities where public health can be involved in policy and plan development on issues relating to healthy community design and tobacco use. Work with local and state decision makers to formulate and implement policies to promote and sustain healthy lifestyles. Coordinate health promotion and public health education programs and activities with other agencies, schools, and professional groups. Works with the MERC committee to assist in promoting a healthy workforce job stress issue, tobacco use, nutrition, and fitness). Tracks and develops workplans and submits quarterly reports as required to ensure appropriate progress on the workplan. Builds internal organizational capacity; collaborate internally to connt DOH-Collier personnel to health trainings and initiatives; engages with community partners; develops partnerships; seeks appropriate grant funding; select, design, and implement strategies; develops effective communication efforts; and conduct evaluations.
Design, support, and facilitate health education by updating program areas on the DOH-Collier website. Prepare and submit promotional and educational materials regarding health promotion and public health education in areas of expertise. Distribute public health messages through Coalition partners, DOH-Collier staff, and through CHO PIO. Solicit vendor quotes for program awareness promotion, prepares purchase requests, and maintains purchase documents from vendors and ensures compliance with state guidelines.
Uses the Performance Management System to develop strategic objectives, track performance and develop action plans to improve underperforming measures. Provides operational leadership of continuous quality improvement activities and participates as an active member of the Performance Management Council. Contributes to the organization's culture of performance excellence by actively seeking to improve personal and organizational performance through measurement, analysis, knowledge management, and processes improvements and participation in performance management committees, councils, and/or workgroups. Demonstrates the Department's ICARE values.

Qualifications:
REQUIRED:
Incumbent must have ability to serve as a health role model and remain tobacco-free while in position.
Valid Driver's License and reliable transportation.
Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

PREFERRED:
A bachelor’s degree from a U.S. accredited college or university, OR three-four years professional level experience in public health promotion, policy development and public health education programs and activities is required.
