Your Specific Responsibilities:
This is work planning, developing, and evaluating the HIV & Hepatitis Prevention Program in the Communicable Disease Control and Prevention (CDCP) Division. This is a responsible, professional Human Services Analyst position requiring the ability to plan, work and act independently, and in a team to meet program’s goals. This position reports to Operations & Management Consultant II. Major program responsibilities include: Provide HIV, Hepatitis and STIs testing and education to inmates at the Collier County Jail; provide referrals upon inmates’ release; assists staff with outreach events, HIV, Hepatitis and STIs screenings and testing, and prevention interventions (Effective Behavioral Interventions); provide linkage to care as prescribed by the HIV, Hepatitis and STIs Guidelines and Florida Department of Health in Collier County Policies in compliance with state, federal and grating agency rules and regulations in determining eligibility and services. Contributes to the organization’s culture of performance excellence by actively seeking to improve personal and organizational performance through measurement, analysis, knowledge management, and processes improvements and participation in performance management committees, councils, and/or workgroups. Continuously demonstrates the Department’s ICARE values. This position is at risk for exposure to Respiratory pathogens (Low Risk Level) under OSHA guidelines and requires annual TB Assessment which may include an Interferon-Gamma Release Assay (IGRA) test; and/or chest x-ray; and is required to have up-to date vaccinations for some vaccine-preventable diseases.
Specific duties include but are not limited to:
- Provide HIV, Hepatitis and STIs counseling and testing to inmates in the Collier County Jail, in addition to clients of the Florida Department of Health in Collier County and to community members during outreach events.
- Provide education, referrals, and linkage to care to inmates in the Collier County Jail, in addition to clients of the Florida Department of Health in Collier County and to community members during outreach events.
- Work collaboratively with Collier County jail staff and Florida Department of Health in Collier County staff to ensure all inmates with communicable diseases receive appropriate treatments and referrals.
- Assists HIV/Hepatitis Prevention Outreach clinics as needed and participates in all National HIV Testing Days.
- Assists in providing group level interventions, and other classes/sessions to target groups as needed to ensure completion of annual Prevention Service Delivery Plan.
- Participate in the quarterly HIV files QA/QI process for completeness and compliance with existing policies, procedures, and regulations.
- Serves as back up of the HIV/HEP Prevention team. Answers phone calls, return phone calls, registers clients from outreach event as needed.
- Participate in all development training programs and/or activities required by Collier County Jail and/or the Florida Department of Health in Collier County
- Participate in bi-weekly prevention staff meetings and completes assigned tasks by due date(s).
- Perform other duties as assigned.
Required Knowledge, Skills, and Abilities:
- Knowledge of the disease process of HIV/AIDS/Hepatitis and STIs, through knowledge of Public Health aspects of patient care at the local level.
- Ability to work with diverse population, counsel and communicate with clients of varying emotional/educational/socio-economic levels regarding sensitive subjects.
- Ability to network with government and non-government originations. Ability to plan, organize and coordinate work assignments.
- Ability to communicate effectively by establishing and maintaining working relationships with others. Knowledge of the principles and techniques of effective communication.
- Skilled and proficient in written communication.
- Proficient in the use of computers and programs; Microsoft Word, Excel, Outlook.
Qualifications:
REQUIRED:
- High school diploma OR two years of professional experience in community services, social, economic, health or rehabilitative services, human resources development and training, program planning, evaluation, research, program analysis, investigative work or business.
- Fluency in English/Spanish or English/Creole.
- Current Florida driver’s license and reliable transportation and willingness to use personal vehicle for business related travel.
- Phlebotomy – Ability to draw blood effectively.
- HIV/AIDS 500/501 Counseling and Testing Certification, 501 annual update and HIV Rapid test certification within 90 days of employment.
Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
