The Outreach Coordinator is a grant-funded position through 2028. The grant can be terminated or extended at any time. The grant may or may not be funded again after that date. Your employment beyond the grant expiration date is not guaranteed. Should the grant not be funded, you are eligible and encouraged to apply for another position within the agency.  The salary for this position is $22.00/hour and is anticipated to work 40 hours weekly. 

 

Your Specific Responsibilities:

  • Assist and collaborate with HIV, Hepatitis and STD prevention services including education, outreach, testing, linkage to care activities, condom distribution, as well as external partners, to build collaborative, sustainable relationships to enhance and expand community engagement initiative.
  • Coordinate HIV, Hepatitis and Sexually Transmitted Diseases (STD) outreach activities at drug treatment centers, mental health clinics, and the homeless population. 
  • Provide HIV, Hepatitis and Sexually Transmitted Diseases (STD) counseling and testing, linkage, referral and education to clients of the Department of Health (DOH) and to community members during outreach intervention and events. 
  • Coordinate and participate in all outreach activities related to community events and health fairs in Collier County.
  • Initiate public health messaging to the Public Information Officer (PIO) in marketing of relevant policy initiatives as requested. Prepare press releases, brochures, and other marketing materials to develop strategic relationships with priority populations.
  • Provide presentation and educational sessions to promote client engagement and retention in HIV care to increase overall rates of viral suppression, based on evidence informed practices, including motivational interviewing, to address identified gaps in care.
  • Promote access to testing, treatment, referrals to STD and Hepatitis screenings, as well as to other medical and social services and link individuals to appropriate care, all to improve health outcomes and reduce disease transmission by conducting on-going community and street outreach in the Collier County area.
  • Develop and establish ongoing collaborations with Faith Based Organizations and local businesses to enhance and expand community engagement initiatives.
  • Collect, analyze, prepare, disseminate reports using program data and on local demographics data to develop results, and present the conclusions to the relevant departments.
  • Collaborate, coordinate, and participate with other DOH staff in the area and other agencies who are responsible for conducting community-based trainings and executing other engagement activities.

 

Required Knowledge, Skills, and Abilities:

  • Excellent oral and written communication skills.
  • Knowledge and experience with infectious disease (HIV, STDs, viral hepatitis) prevention.
  • The ability to establish and maintain effective working relationships with a diverse group of internal and external partners.
  • Excellent Computer skills Microsoft Word, PDF, Outlook .
  • Knowledge of and experience with federal, local, and state policies.
  • Ability to work with managers and staff to identify training needs and research information for training/meeting opportunities, or to provide outreach materials
  • Ability to work with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance use disorders. 

 

Qualifications:

Required:

  • Current Florida driver’s license and reliable transportation and willingness to use personal vehicle for business-related travel.  
  • HIV/AIDS 500/501 Counseling and Testing Certification or certification within 90 days of employment and annual updates.
  • HIV Rapid test certification within 90 days of employment.
  • Proficient in English and Spanish both written and verbal
  • Work hours: 40 hours weekly, 8:00 AM to 5:00 PM.  Additional hours may be required in the event of a public health disaster or as needed.  Ability and willingness to work flexible hours (Evenings, weekends & holidays) when needed to address client needs, outbreaks or other public health emergencies. 
  • EMERGENCY DUTY- Incumbent may be required to work during or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in Special Needs or Red Cross Shelters, or to perform other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

 

Preferred:

  • A Bachelor’s degree from an U. S. accredited college or university in health or science related field preferred or comparable level experience working in an STD/HIV and/or surveillance program.
  • Experience performing phlebotomy.
