ANTICIPATED VACANCY

Your Specific Responsibilities:

This Is a professional nursing position working with the Collier County Public and Nonpublic Schools. Incumbent is to ensure the screening of vision, hearing, scoliosis and growth and development screenings along with follow-up services to students. The incumbent must be able to work independently and is responsible for directing school health staff, including volunteers and other DOH staff assisting with school screenings, and for the planning /set-up of the school screening environment the day of the screening. Annual TB and Immunization screening is required upon hire. This position reports directly to the Assistant Community Health Nursing Director.

Functions as a lead screening coordinator and DOH lead at assigned schools. Functions as the liaison between DOH - Collier staff and volunteers, and the Collier County Public School staff to accomplish required screening services for vision, hearing, growth and development and scoliosis. Assures collaboration with the Public and Nonpublic School Screening Liaisons In each assigned school so screening area preparation and acquisition of support resources are obtained and so that completion of data/documentation elements necessary for student health screenings can be accomplished. Coordinates DOH-Collier and volunteer staff services for the screening day. Participates in the transport and set-up of screening equipment/ environment Performs nursing consultations for children or families with recurrent lice as requested by school staff. Coordinates home visits and offers education and guidance on lice management Oversees all scoliosis screening and ensures students have the resources for proper follow up when screening Is positive for scoliosis. Assists as a Registered Nurse with annual Immunizations back to school event by entering vaccination records or administering vaccines as needed.

In conjunction with Assistant Community Health Nursing Director, functions as a lead team member responsible for compilation of Information necessary for service delivery data, tabulation, and report generation. Assures that methods of acquiring data sets needed to meet the requirements of school health reporting and monitoring In the Health Management System (HMS) are met. Incumbent will be responsible for preparation of documentation needed to conduct screenings, the processing of screening data post screening events, and keeping track of follow up documentation and data tracking for the program throughout the screening season and current school year. Preparing and entering HMS services data for school screening staff, entering screening information Into Health Office Anywhere and FOCUS, and maintain organized files for services and follow up documentation.

Conducts follow-up of referred students found to be presumptively positive for a health condition as Indicated by the results of the screening program. Functions as Department of Health - Collier liaison during the follow-up process. Works with school staff to assure that parent contact is achieved and barriers to access of care are Identified. Communicates referral resource Information with other staff and the student parents or guardians. Processes screening data to generate parent notification documents. Assures distribution to schools as assigned according to the Florida Department of Health Security Polley. Interacts with school screening designee, DOH - Collier team members, parents and others involved in the follow-up process to provide/obtain necessary Information and services. Assures resources for follow-up exams with outside providers for vision. This includes working with the state contracted vision resource to set up vision exams working with Collier County Public Schools.

Assists the Assistant Community Health Nursing Director with data gathering for the annual School Health Report. This Includes upkeep and reports generated from the health management system utilized by the school health program to complete documentation. Maintains the record keeping of the school health screening records Including electronic and paper file records. Also updates and generates all school screening forms, reports, and calendars. At the end of SY, assists with the processing and formatting of data for school district and community partners.

Assists with training and orienting new staff with general office procedures Including but not limited to data entry processes, paper filing system, and travel.

Assists with development of screening calendar and related communication activities with partner agencies such as Collier County Public Schools, Immokalee Technical Institute, Lorenzo Walker Technical Institute, and Florida Vision Quest and Heiken. Participates in the maintenance of equipment used in school health screening activities, assuring that the equipment Is operational, current in calibration and that sufficient supplies are present for program functions. Maintains inventory records. Processes screening data to generate parent notification documents and assures distribution to schools according to the Florida Department of Health Security Policy.

Participates In the Quality Improvement projects. Assists with Kid Care outreach activities such as Health Fairs and Back-to School activities. Enter EARs into HMS according to agency protocols and timeframes. Performs Other Duties as assigned.

Required Knowledge, Skills, and Abilities:

Ability to learn to use HMS.

Ability to use equipment and accurately perform health screenings: vision, hearing, growth and development

Ability to capture and tabulate screening data and track service data

Ability to establish and maintain working relationships with others

Ability to communicate and collaborate effectively with health care providers, community organizations and school personnel.

Ability to function effectively with intermittent supervision.

 

Qualifications:

License per Florida Statute 464. Licensure as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 or possess a nursing multistate license through the Nurse Licensure Compact.

Valid cardiopulmonary resuscitation certification.

 

REQUIRED:

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Valid Drivers License and reliable transportation. 

Must be able to lift 50 pounds.
