At the Specialty Clinic, we are committed to promoting and protecting the health and well-being of all residents. Our Human Services Program Specialist case managers play a crucial role in ensuring that individuals receive the care and support they need.

 

  • Provide medical case management to persons infected and affected with HIV/AIDS.
  • Determine eligibility for the Ryan White Program
  • Complete comprehensive Needs Assessments if individuals presenting themselves for assistance through our agency.
  • Develop individualized care plans to address identified needs
  • Complete and maintain accurate documentation of all client-specific activities
  • Verifies insurance coverages and coordination of benefits. Updates all payor information and refers to (AIDS Drug Assistance Program) ADAP Premium Plus.
  • Provide linkage to care, counseling, support, and education.
  • This position will require travel to our Sarasota location at least once monthly

 

Required Knowledge, Skills, and Abilities:

  • Basic knowledge of Microsoft Office applications, including MS Outlook, WORD and EXCEL
  • Ability to maintain strict confidentiality
  • Relate well to different people from varied backgrounds and different situations.
  • Ability to communicate effectively verbally and in writing
  • Manages a variety of responsibilities in a timely, efficient manner and able to prioritize tasks

 

Qualifications:

Minimum

  • Valid driver’s license
  • One year of professional work experience at a social service or health care related agency or other social services work position
  • Bilingual in English and Spanish
  • Incumbent must complete HIV 500 course and Ryan White Case Management training within 60 of employment.

 

Preferred

  • Associate degree or higher in Social Work, Human Services, Psychology, or Health Science related field
  • Experience with insurance enrollment and coordination of benefits
