Your Specific Responsibilities:

The Health Educator Consultant position is a highly motivated, responsible, and independent position with the Florida Department of Health in Okeechobee County and is responsible for planning, providing, and management of all outreach and health education in Chronic Disease, Healthiest Weight, Injury Prevention, Health Disparities Intervention, and Infant Mortality Prevention. Acts as a liaison for the Office of Minority Health (OMH) with the purpose to support efforts in Okeechobee County to improve health outcomes of priority populations through partnership engagement, planning, and implementation of projects to improve quality of life. Involved within the Performance Management Council for all department strategic planning, continuous quality improvement, and accreditation requirements. Help support functions related to the Community Health Assessment (CHA), the Community Health Improvement Plan (CHIP) and the Community Health Improvement Plan Partnership (CHIPP) meetings.

Undertakes local health promotion and education activities in chronic disease which includes, though not limited to cancer, heart disease, stroke, diabetes, hypertension, and chronic lung disease. Responsible for health education of healthy nutrition and physical activity, injury prevention programs, and other priority health issues. Responsibilities include work in promoting, maintaining, and improving individual and/or community health by assisting individuals and communities to adopt best practices to achieve optimum health and quality of life. Incumbent collects and analyzes data to identify individual and community needs prior to planning, implementing, monitoring, and evaluating programs and/or services designed to encourage healthy lifestyles, policies, and environments. Responsible in submitting various programmatic workplans, reports, and supporting documents in a timely manner based on provided deadlines. Prepares materials and facilitates the Okeechobee County Quality of Life Taskforce meetings. Routinely, participates in community partnership meetings. Develops partnerships within the approved county activities, compile and routinely update a community partner and resource list. Services as a community resource regarding health promotion issues, and data requests. Uses and presents data from DOH CHARTs system and other credible sources to support program planning and implementation. Participates in the Treasure Coast Opioid Task Force (TCOTF) meetings when applicable.

Assists with the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) to protect, promote, and improve the quality of life of Okeechobee County residents. Is familiar with the policies, goals, and objectives of the CHIP in relation to FDOH- State Health Improvement Plan. Helps to support organizational accreditation, quality improvement, performance management, workforce development, and strategic planning activities and participates in Southwest Performance Management Quality Improvement (SWPMQI) meetings.

Adheres to the established Health Department policies and procedures. Attends all conference calls as required and/or directed and completes all required training. Assists in composing newsletter and/or Inside Florida Health content from recently participated community engagement events and activities. Participates in conferences, workshops, interagency committees, and similar activities relevant to the assigned functional area of responsibility.

May assist with researching and/or writing grants to augment health education funding as needed. Serves as a health department representative on various statewide, regional, and community organizations related to health promotion. Attends local and statewide meetings and conference calls as directed. Attends and participates in departmental meetings, training sessions, and conferences to ensure efficiency and effectiveness of operations. Reports to Supervisor to notify of issues or anything that may become a potential problem and recommends changes. Performs other duties and responsibilities as directed by Supervisor and provides information requested by Supervisor and County Health Department Administrator.

Qualifications:

Minimum Qualifications:

  • Florida Driver License
  • Bachelor’s degree in health education, public health, or related field
  • Minimum of two (2) years of full time, professional work experience in closely related field
  • Minimum of two (2) years’ experience in public speaking or providing educational presentations
  • Incumbent may be required to work before, during, and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. Incumbent is required to complete time sheet, work activity reports (EARS), and mandatory trainings.
  • Able to lift and carry 25+ pounds

Preferred:

  • Master's degree in health education, public health, or related field 
  • Bilingual
