Your Specific Responsibilities:

Provider appointments (Adult Primary Care, Family Planning, HIV/AIDS, STD, School Entry Exams and School Sports Physicals):

•           Stay aware of time and timely patient flow.

•           Remind provider with a knock on the exam room door within last 5 minutes of the provider time

•           Calls “no show” clients, documents reason for “no show” and encourage client to reschedule. 

•           Perform Nursing assessments, Triage client needs.

•           Verifies immunization needs for all infants through adults.

•           Record medication reconciliation.  Enter prescription renewal requests.

•           Have client in exam room ready for provider at or before provider time.

•           Assist provider with exam and/or treatment as requested, or specimen collection as needed.

•           Administer and/or issue medication as ordered by medical provider.

•           Perform EKG, vision screening and hearing screening as ordered.

•           Counsel/educate clients regarding treatment plans, health promotion and disease prevention.

•           Verify medications have been ordered, refills provided, and lab orders (including opt-out HIV screening and Hepatitis program eligibility) have been entered prior to client departure.

•           Process referrals and specialty referrals.

•           Properly document client services and codes and interactions in electronic medical record in “real time”.

•           Offer client Continuity of Care Document (CCD) or refer client to “Patient Portal” for CCD.

•           Explain Prescription Assistance Programs (PAP) and client responsibilities.

•           Educate clients regarding prescription renewal procedure with emphasis on the importance of completing ordered labs and scheduling F/U appointments.

•           Assures proper coding of office visits.

•           Verify that PAP and/or ADAP medications are recorded on client medication list.

•           Make sure that client has prescription for PAP medications or alternative until PAP medications arrive.

•           Clean exam rooms with disinfectant between clients.

•           Perform daily exam room set-up.

•           Maintain exam room supply inventory and restock exam rooms.

•           Notify supervisor regarding supply or equipment needs.

Laboratory Services:

•           Able to perform phlebotomy.

•           Perform waved lab tests as ordered.

•           Collects specimens, draw blood as ordered.

•           Prepare specimens for pick-up.

•           Complete all documentation required related to lab work.

•           Maintain lab area cleanliness.

•           Manage all lab services when assigned.

•           Is familiar with lab quality management activities.

•           Clean and sterilize instruments and equipment as needed.

Nursing Protocol Visits:

•           Complete state required training for school physicals and STD Nurse Clinician.

•           Provide nursing services based on local nursing protocols signed by our medical director.

•           Communicate with medical director as needed.

•           Refer for provider follow-up as needed.

•           Make WIC and Healthy Start referrals for positive pregnancy test and as needed.

•           Document all care in “real time”.

•           Enter services/codes prior to client departure.

•           Offer client CCD or refer client to “Patient Portal” for CCD.

Immunization:

•           Able to utilize Florida Shots and documents in “real time”.

•           Able to produce Florida Shots reports as requested.

•           Able to program thermometers, monitor temperatures and complete reports on vaccine refrigerators and freezers as requested.

•           Able to pack vaccines for transport.

•           Certifies Immunization records, 680’s.

•           Accepts delegation to do religious exemptions, 681’s.

•           Administers immunizations as needed to infants through adults.

•           Prepared immunization history and give to parents with education on when next vaccines are due.

•           Completes vaccine reminder calls, emails and letters as assigned.

•           Enter services/codes prior to client departure

Case Management:

•           Case Management to include lab results f/u, telephone call f/u, no show f/u, etc.(other duties as needed to meet client needs)

•           Work Case Management list as assigned.

General Clinic Support Duties: 

•           Document all client communication and care in electronic medical record in real time.

•           Participate in Nursing On-Call duties.

•           Assist as preceptor to new employees and/or students.

•           Registers clients for care.

•           Record daily activities, complete trainings and document as required (EARs, Time Sheets, EDP)

•           Order, receive, stock and inventory medical supplies and medications as requested.

•           Maintaining/servicing/cleaning clinic equipment.  Including sterilization as needed.

•           Emergency box, AED, oxygen and lab alarm checks daily or as required.

•           Maintains clean and orderly workspace.

•           Scans documents as needed in real time.

•           Completes HIV/ADS 500/501 training and yearly updates.

•           Direct to clerical staff all questions regarding charges and billing.

•           Able to open and close (lock and set alarm) the clinic as needed.

•           Serves as a member for meetings and updates staff on changes as needed

•           Assist clinical staff in evaluating TB clients for medication, initiation of TB services and evaluation of TB services.

Professional Development:

•           Attend staff conferences, in-services education programs and seminars to further professional knowledge and

•           Class Specifications of a Lead Sr. Community Health Nurse

Required Knowledge, Skills, and Abilities:

•           Able to use Microsoft Office.

•           Able to use an Electronic Medical Record for real time documentation

•           Customer service skills. Patient Centered Care.

•           Phlebotomy skills.

•           Flexibility

Qualifications:

Minimum –

  • Licensed as a Registered Nurse in the State of Florida.
  • Willing to work with a multicultural economically challenged client population.
  • Utilize an Electronic Medical Record.
  • Competent in the use of Microsoft Word.

Preferred –

  • Bilingual Spanish and/or Creole.
  • Experience with Venipuncture and phlebotomy.
  • Basic Life Support Certification from the American Heart Association or the American Red Cross.
  • Prior clinical experience in Florida Department of Health County Health Department Clinic.
