YOUR SPECIFIC RESPONSIBILITIES:
This is a professional position in the Office of Chronic Disease and Prevention under the Community Health Promotions Division. This Chronic Disease Prevention/Health Equity Liaison Coordinator play a vital role in managing and coordinating various aspects of chronic disease programs and initiatives. The primary responsibility will be to ensure effective coordination and delivery of services aimed at preventing and managing chronic diseases and health disparities within the community. Responsibilities include planning, implementing, and evaluating the new initiatives designed to support the advancement of Chronic Disease prevention, chronic disease self-management and health equality through health promotion, partnership engagement, education and outreach and implementation of focused initiatives. This position will be responsible for working closely with healthcare professionals, community organizations, and individuals to promote health education, support, and access to resources for individuals living with chronic diseases. This position requires strong organizational skills, excellent communication abilities, and a passion for improving the lives of those affected by chronic diseases.
Coordinate and manage chronic disease programs and initiatives, including planning, implementation, and evaluation of interventions aimed at preventing and managing chronic diseases. Stay informed about the latest research, best practices, and advancements in chronic disease management and incorporate them into program development and implementation. Identifying, implementing, and evaluating community engagement models and evidence-based practices that focus on the reduction of health disparities and improve health outcomes (i.e., Health Equity Plan, Health belief model, social ecological model). Establish and maintain a Department of Health-Orange health Equity Team consisting of staff from various program areas, focusing on strengthening the internal infrastructure of the department to support health equity efforts. Identifying and establishing collaborations with critical Health Equity coalition/ Task force members affiliated with population at higher risk and underserved and maintain continuous community engagement. Develops Health Equity Training Materials and assessments for community partners and department staff (i.e., culturally, and linguistically appropriate services (CLAS). Provides technical assistance to internal programs and external to reduce health disparities among racially, ethnically, culturally, and linguistically diverse groups.
Collaborate with healthcare professionals, community organizations, and stakeholders to develop and maintain partnerships and referral networks for chronic disease management. Coordinate with healthcare providers to ensure seamless care transitions, appropriate follow-up, and access to necessary healthcare services for individuals with chronic diseases.
Conduct needs assessments and identify gaps in services to design appropriate interventions and programs tailored to the needs of individuals living with chronic diseases. Developing, implementing, and evaluating policy, systems and environmental changes that have long lasting and community wide impact in the reduction of health disparities; Provides leadership and technical assistance to the Infant Mortality Task Force; the Fetal Infant Mortality Review (FIMR), Florida Healthy Babies, and Florida Healthiest Weight.
Provide education and support to individuals with chronic diseases, their families, and caregivers, including self-management techniques, lifestyle modifications, and available resources. Develop and deliver educational materials, workshops, and training sessions to increase awareness and understanding of chronic diseases within the community. Facilitate support groups and community forums to foster peer support and provide a platform for individuals with chronic diseases to share their experiences and challenges. Coordinates and orchestrates Manages the annual Health Equity Awards efforts and the Minority Health Month awareness activities.
Maintain accurate records, collect data, and generate reports to monitor program outcomes, completion of work plans, identify trends, and assess the effectiveness of interventions. Provides data analysis support for population health projects with specific emphasis on health equity and social determinants of health. Utilize population health data analysis to evaluate health outcomes, patterns of health outcomes in the population (including determinates of health) and the impact of policies and interventions on the health outcome of specific populations or groups.
Attends required monthly and quarterly meetings, to include community and Health Equity Regional partnership meetings. Participate in relevant conferences, workshops, and training opportunities to enhance professional knowledge and skills.
Works with department grant writer to identify funding opportunities and assists in the development of grants to target areas with highest health disparities in the county.

Minimum Qualifications:
- A current Florida Department of Health employee
- A bachelor’s degree in Biological Sciences, Public Health (e.g. epidemiology, biostatistics, population health, healthcare administration, quality improvement), Health Promotion or Community Health Education.
- One year of verifiable professional experience in coordinating a healthcare service.
- One year of verifiable professional experience in conducting health related assessments.
- One year of verifiable professional experience in conducting outreach initiatives and establishing a network of partnerships.
- One year of verifiable professional experience in developing and executing action/work plans to ensure deliverables are achieved.
- One year of verifiable professional experience in collecting, compiling, and analyzing information and data and producing written reports.
- One year of verifiable professional experience in conducting health related presentations to a diverse group of individuals.
- Conduct outreach in the field at various locations throughout Orange County. In absence of a county vehicle, do you have access to personal transportation.
- A valid FL driver’s license or state identification card OR a valid driver’s license from another state (you will be required to get a FL driver license within 30 days of hire).
- Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency. This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
