MINIMUM QUALIFICATIONS:

  • Current Florida Department of Health (FDOH) employee.
  • Willing to work with infectious disease patients (STDs/HIV/TB).
  • Associate Degree in social science or health discipline from an accredited college OR two (2) years of verifiable experience working with people with HIV at an established health services agency.
  • Completion of Medical Case Management AETC Southeast AIDS Education course within 30 days of hire.
  • At least 6 months verifiable experience working with electronic medical records systems.
  • Successful completion of HIV 500/501 course within 6 months of hire.
  • Willing to perform home visits for individuals from diverse backgrounds (varying cultural backgrounds, socioeconomic statuses, and different sexual orientations).
  • Ability to bend, kneel, kneel, use fine motor skills, lift and carry a maximum of 25 pounds.
  • Valid FL Driver’s License and access to a personal vehicle.
  • Emergency Duty.

 

 

PREFERRED QUALIFICATIONS:

  • Eligibility Experience.
  • Experience conducting program assessments and home visits.
  • Current HIV 500/501 certification.
  • Current Medical Case Management AETC Southeast AIDS Education certification.

 

 

 

JOB DUTIES AND RESPONSIBILITIES:

This is a highly responsible position in the HIV Program at the Sunshine Care Center (SCC) of the Florida Department of Health in Orange County. The incumbent serves as a Referral Specialist for the Ryan White HIV/AIDS Program (RWHAP) ensuring all clients of the Sunshine Care Center (SCC) have access to clinical and support services. Incumbent works in partnership with public and private partner agencies and stakeholders, and other HIV/AIDS Service Organizations (ASO’s) including Community Based Organizations (CBO’s). Incumbent reports to the Public Health Services Manager.

 

Staff will establish and maintain a maximum caseload of 150 and provide services in accordance with all Health Resources and Services and Administration (HRSA) and RWHAP service standards. Staff will provide at a minimum six (6) direct client services daily per provider schedule and accurately and timely complete and document all client ncounters in Health Management System (HMS), State, RWHAP systems (Provide), and any other system as determined by the program. Staff will complete financial eligibility determination in accordance with State of Florida requirements for clients of SCC and DOH-Orange. Staff will complete the Referral Determination Assessment (RDA) and refer clients with higher acuity to an Intensive Case Manager within the timeframes established by the service standards, State of Florida, RWHAP, and HRSA policies. Staff is expected to bill a minimum of 540 units each month.

 

Staff will ensure all clients are screened for other payor sources including, but not limited to, Medicaid, Medicare, or other available public or private programs. Staff will verify income, residence, clinical status, recertify, and unenroll appropriate clients in timeframes as specified by HRSA, RWHAP, and State of Florida standards and per those protocols set forth. Staff will demonstrate active collaboration with other agencies to provide referral to the full spectrum of HIV-related or other necessary services, and shall maintain up-to-date knowledge of local, state, federal, and other services available for people with HIV.

Staff will contact clients regarding appointment reminders and adherence to care. Contact may be face-to-face, by telephone, mail, or home visits as per client and program needs. Staff will provide HIV/AIDS health education and advocacy to address medical and treatment adherence, psychosocial needs and paths to access resources. Staff will assist client in determining provider preference and arrange for delivery of coordinated services to meet client’s needs and Service Standards in accordance with HRSA measures (i.e. medical issues/conditions surrounding health). Staff must link consumers to culturally appropriate treatment adherence medical care and work closely with person with HIV to ensure clients have adequate medical services.

 

Staff must maintain subject matter expertise for assigned duties, participate in all training activities and attend meetings as required by the program. Staff will actively participate in clinical care team activities for all clients as directed by the program.
