MINIMUM QUALIFICATIONS

  • Licensure as a Registered Nurse
  • Valid CPR card
  • Must possess a bachelor’s degree from an accredited college or university with a major in nursing or a related field and two (2) years of nursing experience, or have six (6) years of professional nursing experience, or possess an associate degree and have four (4) years of professional nursing experience.
  • Successful Completion of a Level II background screening and drug test.

 

PREFERRED QUALIFICATIONS:

  • At least one (1) year experience working in a long-term care facility with a geriatric population
  • Proficiency with MS Office Suite, especially Outlook, Word and Excel
  • Basic experience and familiarity with MDS 
  • Experience conducting RAI-MDS assessments and compiling data reports based on those assessments
  • MDS Certification

 

POSITION DESCRIPTION:

The incumbent is responsible for scheduling and/or overseeing scheduling of all resident MDS assessments to assure timeliness. Coordinate and oversee all functions of the MDS assessment process to ensure compliance with Federal and State requirements. Reports to the Director of Nursing.

  • Participate in resident level of care determination upon admission, participate in tracking changes in residents’ level of care to assure timely scheduling of MDS assessments and be familiar with residents’ condition and care needs. 
  • Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections. 
  • Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as necessary. 
  • Participate in the Quality Assessment and Assurance Program, in cooperation with the Quality Assurance Coordinator. 
  • Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure this information is included.
  • Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings. 
  • Maintain confidentiality of resident and facility information.  Complete reports and assignments accurately and adhere to established time schedules. 
  • Establish open lines of communication between all departments. Observe infection control procedures relating to Nursing services.
  • Work cooperatively with administration, Nursing Service Personnel, Physicians, community agencies, residents, families, and Consultants. 
  • Perform other duties as assigned by the Director of Nursing Service.
  • Perform all duties assigned in a cooperative, effective, timely, and professional manner.  Be responsible for the safety of residents under his/her supervision.
  • Maintain current certification in Basic Life Support and perform this skill when necessary.
