Minimum Qualifications

  • Valid Driver's License
  • Able to communicate verbally and in writing
  • Willing to successfully complete Disease Intervention Passport Training wtihin 6 months of hire
  • Able to walk one mile or more per day, negotiate stairs or other obstacles, kneel/bend
  • Willint to enter Jaild/Prison
  • Willing to drive your personal vehicle 25% of the time to do field work
  • 6 Months of prior experience in one or more of the following, lab, counseling, emergency services, social work, interviewing, case management or clinical experiences.

JOB DUTIES AND RESPONSIBILITIES:

Conducting Interviews, and re-interviews on clients with STD infections for treatment referrals, and partner notifications.  Documenting each activity conducted with date and time in PRISM System.  Educate Patients regarding STD infections and appropriate medical care.  Interviewing HIV positive clients, referring them for linkage to follow up Medical care.  Prioritizing PRISM Task-list for the day.  Utilizing PRISM System to input STD cases in an accurate and timely manner.

 

Conducting Record search using available resources, such as: HMS, PRISM, DMV, Medicaid, Jail, white pages, Spokeo, medi.net, etc.  Daily clinic duties, when assigned to include, Pre and Post-test counseling, utilizing the Q-Flow system.  Data entry in EHR, Making Field Visits to providers, hospitals, clients’ residence.  Making phone calls to providers, to include Doctor’s offices, Hospitals, Laboratories, and patients to verify treatment and or demographic information.  Performing laboratory duties, collecting urine and blood specimens for testing of these STD’s: Chlamydia/Gonorrhea, Syphilis, Hepatitis, and HIV.  Printing lab labels from E-Lab, preparing specimens for shipment. 

 

Conducting cluster activities and condoms distribution.  Maintaining individual blood kits.  Occasionally conducting Rapid HIV testing on clients. 

 

Creating PRISM profiles for newly reactive HIV clients.  Maintain all necessary logs in SharePoint.  Maintaining state vehicles for cleanliness, gas, and recommended maintenance.  Operating various electronic devices, copy machine, fax machine, printers, Eltrons, computer, etc.  Properly maintaining vehicle mileage log. 

 

Participating in all mandatory trainings/meetings, including passing the STD modules and ISTDI with an 80% score or higher within 6 months of employment.  Stocking and restocking coolers for community screenings.  Transporting clients to and/or from clinic when necessary, providing bus passes.

 

Acting as provider liaison for STD department.  Complying with outlined protocol for outreach activities and departmental security regulations. 

 

Checking emails, and addressing phone messages, returning phone calls.  Resolving clients’ concerns in the clinic if possible, or consulting with supervisor

 

Performing other duties not considered routine to support the mission of the program, Department, and agency.

 

KNOWLEDGE, SKILLS AND ABILITIES, INCLUDING UTILIZATION OF EQUIPMENT, REQUIRED FOR THE POSITION: 

Ability to communicate verbally and in writing

Ability to conduct Health investigations

Ability to conduct fact finding interviewing

Ability to perform phlebotomy

Ability to analyze data

Knowledge of methods of compiling organizing and analyzing data

Ability to prepare records and reports

Ability to maintain confidentiality

Ability to establish and maintain effective working relationships
