This Health Support Technician position serves in a professional capacity, providing clerical services for the School Health and Clinical Programs, in addition to serving as a community education and outreach coordinator in Putnam County. The incumbent will receive cross-training in client services to serve as a back-up clerk. It is a responsible and flexible position, which requires independent judgment in a variety of settings. The work will be performed under the direct supervision of the Senior Community Health Nursing Director and take direction from the School Health Program Specialist Registered Nurse as needed.  

Coordinates School Health Screening and annual Full Time Equivalent Week and Compulsory Immunization reports. Assists in providing FSS Basic School Health Screenings for vision, hearing, and scoliosis; maintains records of screening results; tracks parent contact attempts regarding abnormal results; tracks referral outcomes on health screenings; mails letters as needed.

Coordinate's immunization clinic schedule with school administration; Monitors cumulative health records checks for compliance with immunizations and FL Physical examination; Tracks and maintains spreadsheet for students out of compliance at all schools; Responsible for processing data, ensuring timely letters are complete for non-compliant cumulative health records.
Conducts annual QA audits for all school health rooms; Completes and submits QA audit report to supervisor. Coordinates "Florida Youth Survey" countywide, for data collection and entry; Assists School Health Coordinator with delegated duties.

Coordinates and executes School Health outreach to increase compliance, including but not limited to:

-    Immunization clinics
-    Health education classes
-    Chronic disease management
-    General program information 


Provides general clerical support for School Health program:

-    Maintains a current roster of students with chronic diseases in all schools
-    Assists with care plans as delegated by School Health Coordinator
-    Ensures contact numbers for school administration are current and readily available
-    Performs weekly HMS data entry for all schools    

Regularly plans and participates in community outreach:
-    Actively cooperates with community agencies or organizations to facilitate and maintain optimal working relationships
-    Identify and develop collaborative resources to accomplish public health initiatives
-    Represents clinic programs at community organizations, meetings, and events to promote services
-    Coordinates and participates in outreach events to provide community education
-    Documents activities, events, and outreach efforts and reports to supervisor monthly

Performs general clinic support services as directed, to include but not limited to:

-    Obtains and documents vital signs, height, weight, and medical history
-    Phlebotomy and specimen collection services
-    Prepares exam room and clients for provider exam 
-    Assists provider with bedside procedures
-    Records data and services in EHR
