Provides PS to a wide variety of individuals infected with or at-risk of a STDs (as identified under duties and responsibilities above) to elicit partner and suspect information. Uses information for epidemiological analysis to identify and the source and spread of cases related to the known infection(s).
Conducts field investigations to locate and provide Field Directed Therapy to individuals infected with
Chlamydia/Gonorrhea and/or to refer individuals infected with or suspected to be at-risk of infection for STDs for counseling, medical evaluation and/or treatment.
Performs case management activities as directed in the process of discovering and/or identifying infected individuals. These activities will include but are not limited to work prioritization and records documentation and management, clustering, HIV pre- and post-test counseling, visual case analysis and epidemiological techniques, data entry and/or other related activities.
The DIS expanded duties include the processing over 75% of all incoming STD surveillance laboratory reports. For each lab report received, the DIS must first conduct a record search in the PRISM data management application to determine whether clients have been treated for the STD they have tested positive for. Most of the reports, however, require the DIS to make contact with the providers to determine symptom history, whether the client received the appropriate medication, and to arrange for face-to-face partner notification services. These expanded responsibilities, coupled with their current duties (client interviews to identify exposed sex partners, field investigations to locate difficult to find clients, field phlebotomy, community-wide educational presentations, etc.), is an enormous challenge that the Area 2 DIS continue to live up to. Incumbent also has been designated to oversee the Linkage to Care Program in certain counties in Area 2.
Completes required records and reports including case management forms and laboratory request forms, etc. in accordance with current CHO and STD Program policies and/or procedures. Provide Rapid HIV and Syphilis testing to clients in assigned area. HIV test results will be provided to the 24 hours after receiving the test. In collaboration with the universities, performs pre- and post-test counseling for confidential Rapid HIV testing clients.
Provides technical assistance to Provide Medical Center on matters of HIV/AIDS and STD program services, planning and screening efforts. Communicates with physicians, clinic and laboratory personnel to confirm diagnosis, treatment and laboratory results.
Communicates with and visits public and private medical doctors, clinicians, hospitals, laboratories and other health care providers to (1) obtain demographic, medical or other specific client information including address, telephone number, place of employment, social security number, race, gender, date of birth, test results, diagnosis, treatment, etc. and/or to arrange medical evaluations and treatments for a client; (2) stimulate reporting of cases and/or positive reactive test results in accordance with FL statutes and administrative rules; (3) acquaint practitioners with current diagnostic and treatment guidelines, disease trends and their role in reducing the rate of infection in the community.
