Your Specific Responsibilities:
This is a highly responsible professional position with lead responsibility for coordinating health gap intervention activities in Bay County. The primary responsibilities of this position include mobilizing partners to advance minority health and address health gaps as they are related among the populations at a higher risk and that are underserved. This position is responsible for building community capacity that includes traditional public health organizations and nontraditional partners to reach disproportionately affected populations with tailored programs and practices for healthcare strategies.
Convene and facilitate a multi-sector minority health coalition that includes members of underserved communities and organizations that serve the community. Engage partners and building updating minority health plans with the intention of establishing systems that can also support future emergency responses.
Build cross-sectional partnerships to align public health healthcare and non-health interventions that decrease health gaps and inequities. Support county activities and outreach to increase access to health-related services to populations that are underserved.
Identify and establish collaborations with critical partners affiliated with and who provide services to populations that are underserved to disseminate accurate culturally and linguistically responsive information and facilitate access to health-related services. Work with local faith-based, education, health and social service agencies and other community gatekeepers to increase participation and community outreach, data collection and emergency planning.
Assess training needs of County Health Department staff and other community partners to improve their capacity addressing socioeconomic gaps. Coordinate training activities with regional coordinators assigned to Bay CHD.
Oversee administration and reporting of county minority health work plan and budget period attend minority health trainings.
Assists with the development and analysis of the community health assessment the community health improvement plan and the county strategic plan and in accordance with the National Association of county and city health standards.

Required Knowledge, Skills, and Abilities:
Working knowledge of Microsoft Office. Experience working with community schools, businesses and organizations.
Preferred: Must be able to stand long periods of time and lift a minimum of 25 lbs.
