This position is responsible for developing and implementing activities associated with active surveillance (field investigation of suspect and confirmed cases) of HIV infection and Acquired Immune Deficiency Syndrome (AIDS) that meet the Center for Disease Control (CDC) Reportable case criteria for these diseases. This position has responsibility for HIV/AIDS surveillance throughout Area 1, Florida (Escambia, Santa Rosa, Okaloosa, and Walton Counties) and functions under the general supervision of the Community and Social Service Manager 1 - SES. This position requires knowledge of health care services (both public and private), Florida law, counseling and interviewing techniques, principles and methods of Epidemiology, and the ability to collect, compile and analyze data derived from surveillance activities. Serves as Information Custodian. The Surveillance Coordinator is also responsible for researching and developing information appropriately formatted for distribution to the media/public, health professionals, health educators, and individuals or agencies applying for federal, state, and private funding, while working closely with the Public Information Officer (PIO) and/or communications department representative before releasing any information. This position is also responsible for providing ongoing support and surveillance training to personnel participating in the reporting of HIV/AIDS cases. The surveillance coordinator must be able to work proficiently with data collection and surveillance computer software programs to include HIV/AIDS(HAL) and STARS reporting systems. Effective communication skill and the ability to work closely with both health care providers and persons living with HIV/AIDS, is essential. The surveillance coordinator is the Information Custodian of the HIV/AIDS Surveillance Program and as such is responsible for knowledge of and confidentiality of HIV/AIDS information. This position reports directly to the Community and Social Service Manager 1 - SES and is assigned to the HIV/AIDS and Linkage Programs. Travel may be required.
Establishes and maintains active liaisons with medical institutions and private providers who provide health care services to clients. Physicians or Health Care Providers who order HIV test and return seropositive are contacted on a case-by-case basis to explain and offer follow-up services by the Department of Health. In addition, professional support is given to assist with reporting activities by telephone, through the mail, and on-site visits.
Serves as Information Custodian. Assures compliance with current Department of Health security policies by fulfilling the Information Custodian responsibilities described in Security Policy 3, areas of responsibilities. Ensures that data set(s) under incumbent’s responsibility are available to those who need to know, protects the integrity of assigned data set(s), and assists the Security Coordinator as needed to comply with policies and procedures
Coordinates and directs data collection activities for Area I and ensures data entry, data management, quality control and timely transfer of data to program headquarters in Tallahassee. Completes timely periodic reports required by headquarters. Also provide assistance by disseminating non-confidential data to persons/agencies who have a need for the information.
Collaborates, advises, and assists Area I county health department directors and program managers with HIV/AIDS cases reporting and issues relevant to follow-up activities with clients including counseling/testing, HIV/AIDS information and referral and coordination of voluntary partner notification activities.
Serves as the Linkage Coordinator for Escambia, assist with linking individuals to medical care within 30 days or less after diagnosis. Maintain documentation of client interaction and outcomes. Give additional counseling and education as needed.
Assist with the Test and Treat services (TnT). Determine eligibility, lab draws and made appropriated community referrals. Ensure all client’s data is entered into the Health Management System and STARS.

Qualifications:
Minimum:
High Diploma
0-1 years of work experience
Employed with the Florida Department of Health.
Current work experience with Surveillance Computer Software (STARS and HIV/AIDS Labs (HAL).
Preferred:
Experience interpreting Clinical Labs.
Experience collecting, compiling and analyzing statistical data.
Venipuncture experience.
500/501 Certification.
