Your Specific Responsibilities:

This professional position will serve in multiple capacities for the implementation of projects and initiatives for the Florida Department of Health in Charlotte County.  This position is a Community Planner that will serve in the role of community health planner to improve community health. The incumbent in this position will assist and participate in specific assigned activities related to building and maintaining community partners’ relationships, Community Health Improvement Plan (CHIP), Community Health Assessment (CHA), implementation and evaluation of community prevention interventions. This position is also responsible for the planning and analyzing of data for the CHIP, CHA, and other organizational plans and reports.

 

This full-time position is under supervision of the Program Manager and must be able to work independently.  

 

PHAB Accreditation: Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)

  • CHA:
    • Prepare and lead Health Assessment efforts
    • Lead exercises, planning activities, focus groups, preparation, and updates of the CHA
    • Engage community members and organizations to participate in and contribute to Healthy Charlotte & the CHA process.
    • Analyze results of surveys to aid in the development of the CHA
    • Collaborate with internal and external stakeholders to determine community health needs and health disparities
    • Develop, write, create, and maintain the CHA final report/assessment.
    • Prepare documents and reports needed to meet Public Health Accreditation Board (PHAB) requirements
  • CHIP:
    • Utilize data obtained from the CHA and collaborate with stakeholders to develop, update, and implement the CHIP
    • Serve as the CHIP coordinator and lead the following: DOH-Charlotte liaison to Healthy Charlotte, identify evidence-based initiatives and/or best practices for identified priorities, track next steps and action items, develop annual progress reports, provide administrative support.
    • Engage community members and organizations to participate in and contribute to CHIP
    • Work with community partners to obtain buy-in for proposed programs and initiatives
    • Lead meetings, events, and activities that support CHIP priorities, goals, and objectives
    • Oversee and prepare the CHIP final document  
    • Ensure alignment of the CHIP with national, state, and local priorities and objectives
    • Manage and report quarterly and yearly performance measures
  • PHAB Accreditation:
    • Assist with all re-accreditation efforts.
    • Ensure timely submission of documentation for accreditation and re-accreditation.  
    • Assist in the identification of gaps or deficiencies in the accreditation standards and measures.
    • Participate in trainings, webinars, and meetings related to accreditation.
    • Prepare for possible operational site visits.
    • Assist with the preparation of associated required reports
  • Performance Management
    • Utilize the organization’s performance management system for monitoring and reporting.
    • Prepare reports to measure impact of initiatives on the public health system.
    • Assist with preparation of necessary documents (i.e., agenda, reports, presentations, minutes). 
    • Assist with preparation of performance score cards for multiple plans and programs, to include the Strategic Plan, CHIP, Quality Improvement (QI), Workforce Development, and others.
    • Support Strategic Planning efforts by participating on the Strategic Planning workshops and exercises, providing data, and tracking measures of success.

 

Community Collaboration:

  • Participate in various functions, meetings, and activities to ensure DOH representation, participation, and collaboration with community partners. 
  • Work with the Administrator, department leaders, local government, and community partners to participate and promote public health initiatives.

 

Quality Improvement

  • Work with supervisors identifying opportunities for quality improvement, participate in Quality Improvement (QI) initiatives, and provide guidance to staff on the use of the PDCA methodology and completion of QI projects.
  • Lead and assist in various trainings and exercises.
  • Assist Department Director in preparing documents as required

 

Data Analysis:

  • Collect and provide program data to program leads as requested.
  • Research, review and interpret health data to inform plans, initiatives, and decisions.
  • Gather information from various sources, interpret patterns and trends.
  • Create reports that detail the conclusions developed from that data.
  • Identify opportunities for improvement using data trending.
  • Develop graphical presentations of data that can be used to illustrate findings clearly.

 

Other Duties:

  • Coordinate scheduling meetings and trainings with identified project leads for multiple initiatives.
  • Attend team meetings, in-services, and trainings that support professional development
  • Organize preparation of written and oral reports and presentations that communicate necessary information to professional audiences, policymakers, and the public.
  • Design, distribute, collect, and analyze results of surveys for various functions.

 

Qualifications:

  • Required:  A valid Florida driver’s license
  • Preferred:  Work experience in public health
  • Preferred:  A bachelor’s degree from a U.S. accredited college or university
  • Preferred:  A bachelor’s degree or higher from a U.S. accredited college or university with a major in health education, community or public health, or behavioral sciences
