Your Specific Responsibilities:

This is a highly responsible position in the Healthy Start Program of the Franklin and Gulf County Health Departments, working to reduce infant mortality rates and improve pregnancy and related health outcomes. This position will be directly responsible for intakes, care coordination and case management activities of the Healthy Start Program. It is a sensitive position that is responsible for the integrity and confidentiality of data sets under his/her control and is under the supervision of the Senior Human Services Program Specialist-SES.  Frequent travel is required throughout Gulf and Franklin Counties.

Provides comprehensive care coordination and home visiting services to women, children, and families enrolled in the Healthy Start Franklin-Gulf program.  This includes - but is not limited to - comprehensive initial assessments, case management, child developmental screenings and education, mental health screenings, prenatal and postpartum education, breastfeeding education and support, tobacco education and support, inter-conceptional counseling, assisting families with goal setting, and linking families with community resources/services as outlined in the Healthy Start contracted deliverables and Standards and Guidelines.  Documentation of all services provided is completed in accordance with the Healthy Start contract and Healthy Start Standards and Guidelines.

Conduct intakes and assess health care and social service needs of incoming prenatal and infant/child CIR/Healthy Start referrals for eligible women and children in accordance with the Healthy Start contract and Healthy Start Standards and Guidelines.

Works collaboratively with the Healthy Start Coalition, Healthy Families Franklin/Wakulla/Gulf program, WIC, other community programs and resources, prenatal & pediatric providers to ensure services are being provided to maximize families’ health, well-being, and self-sufficiency.  This includes attending community meetings, networking events, and community education forums to promote relationships and develop resources for the program.

Serves as Car Seat Technician Proxy for Franklin and Gulf health departments.  Maintains car seat technician certification.  Participate in staff meetings, Coalition meetings, trainings, on-site monitoring of the Healthy Start Franklin-Gulf program and attends the Fetal and Infant Mortality Review (FIMR) case review meetings. 

Performs other related duties as assigned by the Sr. Human Services Program Specialist-SES or DOH Franklin/Gulf Administrator.

 

Required Knowledge, Skills, and Abilities:

Judgement and decision-making abilities; good problem-solving abilities; knowledge of community resources and the referral process; ability to work and communicate effectively with people using a strength-based approach; excellent verbal and written communication skill; basic knowledge of computer software and applications.

Qualifications:

  • Valid Class E Florida Driver's License required
  • At a minimum, a high school diploma or equivalent (required by Healthy Start contract)
