Your Specific Responsibilities:
This is a Senior Community Health Nurse Position that works as an immunization coordinator. This position provides services in the primary care/immunization clinic and in Community settings. Work is performed under the direct supervision of the Assistant Community Health Nursing Director.
Update records in Florida Shots, Process 680 requests, Complete College Vaccine Forms, PPD Testing, HMS visit/Screening for TB Gold Lab Tests. Employee Vaccinations, Religious Exemption Vaccine Education, and issuance of Religious Exemption Forms. Schedule Return Vaccination appointments, provide Back up for EPI clients (Tdap, Rabies vaccines, Hepatitis A and Hepatitis B vaccines) and assists with outbreak response.
Provides direct patient care to clients served by the county health department. Administers medications and immunizations in accordance with the agency protocol. Performs routine diagnostic tests, medical treatments, taking and recording temperature, pulse, B/P, heights and weights, eye exams and hearing screens. Interviews, counsels, and provides health education in accordance with the local county health department guidelines. Provides documentation to and updates patients’ charts. Reports any unusual symptoms of patient to supervisor and provider.
Provides vaccines to WIC clients at the Lake Whitney office on 531 NW Lake Whitney Place, Port St. Lucie, vaccines to clients at the clinic on 1701 S 23rd St. Fort Piece, FL, and other FDOH-St. Lucie clinics as needed.
Functions as the primary contact for community agencies. Promotes vaccine adherence and education in community settings and conducts educational presentations as requested or needed autonomously. Promotes employee vaccinations and set-up outreaches with agencies throughout St. Lucie County and upon request. Organizes and provides outreach vaccination services. Drives company car to outreaches throughout St. Lucie County. Participate in Back-to-school summer events. Ensures all MOAs, agreements, and supplies are up to date prior to providing outreach service.
Orders and receives all vaccines and maintains vaccine inventory through physical counts and FL shots. Manages VFC training certificates for employees and monthly inventory checks. Receive inventory, apply inventory to FL Shots, and stores vaccines for EPI and TB Clinic. Maintains vaccine refrigerator temperature logs and monitors for discrepancies. COVID vaccine management. Maintain Ultra-Freezer temperature logs.
Maintains a clean and safe work environment as it relates to state guidelines. Adheres to client confidentiality, universal precautions and county health department guidelines as defined in the guidebook and local policies. Communicates regularly with supervisor. May be required to work more than an 8-hour day or weekend with flextime to be provided.
Assists with ordering and receiving medical and clinic supplies, maintaining inventory management of medical/clinic supplies, stocks and distributes supplies to clinic rooms as needed. Participates in nursing meetings and CHD meetings. Attends local trainings as scheduling allows and information is work related.
Required Knowledge, Skills, and Abilities:
One year nursing experience, Able to stand for 2 hours uninterrupted, lift 40 pounds, walk half a mile, Florida driver’s license.
Qualifications:
Minimum:
- One year experience working in the nursing field.
- Valid Driver’s license, if out of state, a Florida Driver's license is required within 30 days of hire
- Current, active Florida Registered Nursing License or Multistate Registered Nursing License from another compact state
- Willing and able to drive your own vehicle or a state’s vehicle to outreaches around St. Lucie Country
