Your Specific Responsibilities:
This position will work as the frontline public health worker for our patients in the community. This position will serve as a liaison between the health services and our community to facilitate access to quality service and health care delivery. Incumbent will have the ability to provide a shared sense of vision across organizational boundaries to drive collaborative action and resource-sharing. Incumbent must be efficient in multi-tasking, which includes, prioritizing-ability to assess your responsibilities and organize your tasks in terms of importance, delegating-ask others for assistance to complete tasks to keep the project on schedule, time management-planning your time to ensure you meet deadlines and goals. Incumbent will be dedicated to the full-time delivery of services, including all administrative and data analysis tasks.
- Provides daily consultation and assistance to management and staff in the management and operation of service programs and responsibilities.
- Evaluates data for program planning and decision making.
- Develops and maintains a community partner referral system.
- Serves as the subject matter expert in linking clients to identified clinical and/or social services in local and surrounding counties.
- Establishes partnerships with community stakeholders, organizations, lay persons and entities to address health priorities.
- Confers with medical professionals as needed.
- Recommends operation policies and assists in the establishment of procedures, as required, to implement policies, achieve operational objectives, or resolve operational problems.
- Monitors operation activities to meet specific measurable service objectives to assure compliance with statewide operation goals.
- Develops tracking system to monitor and assess health outcomes of program participants.
- Effectively communicates complex information.
- Other duties as assigned by management.
Required Knowledge, Skills, and Abilities:
- Ability to function effectively as a team member, or leader in one’s activities.
- Ability to communicate complex information effectively both verbally and in writing to patients, families, staff, and providers.
- Ability to develop a care/intervention plan, implement the care/intervention, and evaluate the intervention adjusting as necessary.
- Knowledge of health promotion, disease prevention, preventive services, and chronic diseases including physical, behavioral, social, and financial considerations.
- Knowledge of community resources to assist patients in meeting their needs.
- Knowledge of the methods of data collection and analysis.
- Ability to collect, evaluate and analyze data to develop alternative recommendations, solve problems, document workflow and other activities relating to the improvement of operational and management practices.
- Ability to organize data into logical format for presentation in reports, documents, and other written materials.
- Ability to conduct fact finding research.
- Ability to utilize problem-solving techniques.
- Ability to plan, organize and coordinate work assignments.
- Ability to work independently.
- Ability to establish and maintain effective working relationships with others.
- Knowledge of Microsoft office.
- Bilingual preferred
