This is a highly independent and responsible Health Educator Consultant position with the Department of Health Seminole to support the outreach team. This position is part of a multidisciplinary team that ensure proper care and treatment to clients who are seen for the following services: Asthma Home Visiting Program and BCCEDP support.
Duties and Responsibilities
Under supervision of the Executive Community Health Nursing Director this position is responsible for initiating and coordinating Asthma Home Visiting Program and supporting the Breast and Cervical Cancer Early Detection Program. This position will be focused on activities associated with Asthma education and prevention. Responsibilities include providing individual and/or group asthma education to clients referred to the program as well as home visits to identify and abate environmental trigger and promote the Asthma-Friendly Schools and Childcare Recognition Award Programs and provide mentoring services for Seminole County schools as necessary. Education focuses on understanding prescribed medications, proper inhaler utilization, and assessing and addressing environmental triggers, allowing patients and families to better self-manage the asthma. The incumbent will also help address gaps in care for the patient population as related to asthma and should be able to demonstrate knowledge and application of health promotion, understand behavior change strategies when working with patients, and understand culturally competent health communication strategies.
This position conducts asthma environmental home assessments and works with families to identify and implement practical steps to reduce asthma triggers in the home. It provides instruction on proper inhaler use, delivers individual and group asthma education, and evaluates the effectiveness of education during follow up visits. The role maintains electronic records, compiles statistical data, and ensures all documentation is completed accurately and on time. The consultant stays current on asthma best practices through ongoing professional development and ensures educational materials are kept up to date and readily available.
The position consistently demonstrates professionalism, strong customer service, flexibility, and teamwork while managing time effectively to meet program goals. It collects program data, prepares required reports, and contributes to overall program performance.
This role assists with developing public health education materials and provides presentations to community groups and partners. It supports outreach efforts by planning and conducting activities such as community partner engagement, participation in community center programs, health fairs, events, and targeted screenings. The consultant represents the Florida Department of Health in Seminole County at work groups, trainings, and community outreach activities, ensuring effective education and engagement across diverse populations. Collectively develop a work plan, budgets, purchasing goods and services for as it relates to each policy area.
Perform other duties as assigned.
Assists Breast and Cervical Early Detection Program Coordinator and/or designee with the coordination of scheduling, screening, and processing qualified clients to receive services.
Enter all client encounters – both face to face and by phone into the Health Management System (HMS) and other software tracking systems for reporting purposes. Submits required reports, maintains assigned records, completes/returns all paperwork pertaining to job duties accurately, legibly and in proper order. This would also include timely submittal of timesheets, mileage, and EARS. Answers incoming telephone calls in a timely manner; speaks clearly and concisely, exercising tact and providing accurate information; accurately and efficiently provides support in other clerical areas as needed; interacts positively and professionally with nurses and other clinic staff to assure client needs are met.
Assist with chart reviews and other QA/QI activities to improve clinic functions and to meet DOH-Seminole’s mission statement and goals. Participate of the Quality Advisory Committee, maintaining appropriate relations with internal – external partners.
Participate in team meetings and all meetings convened by the Asthma Program and DOH community as deemed necessary.
Conduct outreach activities for potential clients to educate and promote the availability of services. Incumbent will participate in health fairs, community events and collaborate with providers among other activities.
This position may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disasters or threat of disaster, man-made or natural. This position will maintain confidentiality and security of records as specified by departmental regulations and laws.
Required Knowledge, Skills, and Abilities:
Knowledge, skills and abilities, including utilization of equipment, required for the position: Ability to conduct presentations, home visits, visits with community partners and Providers to provide education about the program. Customer service skills. Ability to prepare reports, understand and apply applicable rules, regulations, policies and procedures relating to State and Local Programs, understand and follow legislative policy direction as established in Federal and Florida Statutes; deal with the public in a tactful and courteous manner; work independently, plan, organize and coordinate work assignments, communicate effectively, establish and maintain effective working relationships with others; utilize problem solving techniques and use personal computers
Qualifications:
Minimum –
Verifiable work experience scheduling appointments, tracking services, and maintaining reports in an electronic database
Preferred –
Are you willing to conduct In-home visits for assessment and educational purposes for pediatric clients and their families as needed.
