Your Specific Responsibilities:
This is a clinical services position at the Florida Department of Health in Walton County. This is a role providing practical nursing support and assistance to providers primarily in the laboratory but may assist in other clinical areas as needed in accordance with agency policies, procedures, protocols, and guidelines. This position is a key custodian for the medication room, laboratory and medical records room. Adheres to Information security policies and procedures. This position is based at the Santa Rosa Beach site but may be assigned to other clinic locations as needed for staff coverage.

This position will assist in the provision of care coordination:

- Links patients with community resources to facilitate referrals and respond to social service needs.
- Tracks and supports patients when they obtain services outside the practice.
- Obtains tests and referral results and communicates with community organizations, health plans, facilities, and specialists.
- Communicates test results and care plans to patients/families.
This position will follow Florida Department of Health, Florida Department of Health Walton and Walton Community Health Center policy and procedures.
Practical Nursing Tasks
Laboratory Duties
• Collects laboratory specimens per provider’s orders on children and adults (blood, urine, sputum, stool, culture swabs)
• Labels specimens at time of collection
• Enters client data for lab order into HMS for processing and ensures services are captured on the superbill
• Stores specimens according to laboratory requirements prior to shipping
• Ships laboratory specimens to outside labs per lab protocols
• Use and maintenance of lab equipment including centrifuge, autoclave, glucose machine, hemoglobin machine, HgbA1C machine
• Tracks lab for results return and notifies provider of abnormal results
• Follows up on lab specimen results that are not resulted timely
• Utilizes personal protective equipment in the clinical and laboratory setting as required by policy and procedure
• Utilizes aseptic technique for lab collection procedures
• Is responsible for daily laboratory QA/QI duties including controls and checking storage temperatures
• Is responsible for Maintenance of and QA/QI for Autoclave
• Maintains testing logs and manifests
• Responsible for reviewing and reconciling the E-labs Error Report daily for all sites
• Maintains HMS E-lab system to include updating, tests, codes, prices and collection/shipping requirements
• Performs AAFP/PT at all sites and maintains documentation for this process
Documentation
• Record Objective and Subjective Data in Electronic Health Record as required for each designated program
• Record Objective and Subjective Data before the end of the clinic visit
• Perform Chart Review as directed for QA/QI
• Documents lab results in appropriate lab binders and in HMS
• Maintains all required laboratory logs
Facility Maintenance
• Obtains supply inventory for lab
• Orders and stocks needed supplies in lab
• Cleans lab draw room and direct patient care equipment after each visit and ensures lab draw room is client ready before each visit
• Empties trash and disposes of supply boxes and or containers
• Secures full sharps containers, stores full sharp containers in designated areas, and replaces containers as needed in clinic areas
• Cleans spills immediately when observed or discovered
• Immediately reports non-functioning or broken equipment to direct supervisor
• Performs crash cart inventories for clinic and dental on a monthly basis
Direct Client Care
• Performs Hearing Screening
• Performs Visual Acuity Screening
• Performs EKG and other in- house diagnostics
• Assisting Provider with bedside procedures
• Administers oral, intramuscular, topical, subcutaneous, and inhalation medications and administers vaccines
• Provides educational counseling and educational brochures to clients as indicated by health care needs
Triage and Work Up (infants, school aged children, adults)
• Obtains and documents Vital Signs, Height, Weight, Body Mass Index (BMI)
• Obtains Chief Complaint/History of Present Illness, Obtains Medical History, Allergy History, Medication History and reconciles Current Medication List
• Prepares client/ exam room for provider visit
• Reports abnormal findings to provider
Leadership Role
• Responsible for training new employees/staff on lab processes and procedures.
• Resource person for testing information for all staff
Other
• May perform other duties within the practical nursing scope of practice.
• Attends and participates in monthly general staff meetings and departmental meetings.
• Completes all job related assigned trainings.
• May participate on teams and/or committees with approval from supervisor.
• Completes Employee Activity Reports (EARS) and submits timesheets according to agency standards
• Performs other related duties as assigned.

• Ability to use and maintain clinic equipment such as the centrifuge, autoclave, EKG machine, nebulizer, pulse oximeter, manual and digital scales, stadiometer, audiometer, vision screening machine, glucose machine, eye wash station, electronic blood pressure machine, manual blood pressure cuff, and hemoglobin machine.
Qualifications:
Minimum - Licensed LPN, valid driver’s license, CPR certification, HIV 501 certification
Preferred – Previous Public Health experience
