Your Specific Responsibilities:

This is a high-level professional position responsible for Community Health Improvement activities and projects in the office of Community Health within the Department of Health in Sarasota County.  This position is responsible for the Community Health Assessment, Community Health Improvement Plan (CHIP), and projects related to the CHIP. Additionally, this position is key to quality-of-life work of the agency and serves as the Minority Health Liaison for the Office of Minority Health and Health Equity (OMHHE). Must be tobacco and nicotine free. This position reports to the Program Administrator-SES of the Florida Department of Health in Sarasota County.

 

  1. Community Health Assessment and Improvement Plan
    1. Responsible for coordinating activities related to the Community Health Assessment and producing a report for the community as required as least every five years
    2. Coordinates community planning and implementation of Community Health Improvement Plan projects related to the Sarasota County Community Health Assessment
    3. Writes time specific and measurable goals and objectives for the Community Health Improvement Plan related to the health needs identified in the Community Health Assessment
    4. Engages key community stakeholders in the planning and implementation of the Community Health Assessment and Plan
    5. Works with health promotion and public health education staff to plan, implement and evaluate health promotion programs based on community needs
    6. Responsible for tracking activities outlined in the Community Health Improvement Plan and reporting actions regularly to key internal and external stakeholders
    7. Completes special assignments such as preparation of annual reports, strategic plans, etc. and preparation, implementation and evaluation of annual plans, goals and objectives related to the Sarasota County Community Health Assessment and Improvement Plan
    8. Assures State guidelines are followed, and reports are submitted in a timely manner
    9. Reports quarterly progress on plans to the Performance Management Council
  1. Quality of Life
    1. Serve as agency’s expert on quality of life and is the Minority Health Liaison
    2. Attends state and local meetings and training on quality of life and minority health
    3. Responsible for the Quality of Life Plan and routine and required updates
    4. Responsible for the annual report and other reports required by the OMHHE
    5. Coordinates the Quality of Life Team, Quality of Life Taskforce, and Quality of Life Coalition including meeting notices and meeting summaries
    6. Assures quality of life is incorporated in project planning and implementation with internal departments and through community projects and program
  2. Community Health Improvement Partnership (CHIP) Leadership Council
    1. Engages and provides updates to the CHIP Leadership Council related to the Community Health Assessment and the Community Health Improvement Plan
    2. Represents DOH-Sarasota at community meetings and serves on councils, boards and task forces related to the Community Health Assessment and Improvement Plan as needed
    3. Provides assistance related to activities of the CHIP Leadership Council meeting and Healthy Sarasota County program initiatives
  3. Miscellaneous Activities
    1. Attends team meetings, DOH-Sarasota staff meetings and participates in staff development activities as required
    2. Maintains client confidentiality in a manner consistent with Florida Statutes and DOH policy
    3. Performs related duties as necessary with adjustable scheduling
    4. Provides support for other initiatives such as Healthy Communities, Community Health Action Teams, and Tobacco Use Prevention

 

Required Knowledge, Skills, and Abilities:

  • Ability to independently prepare written materials

  • Ability to effectively communicate with staff and the public
  • Ability to interact appropriately with others and maintain effective working relationships
  • Ability to adjust to changes in work environment and responsibilities
  • Ability to operate various machines and equipment such as word processors, computers, typewriters, copiers, and fax machines, etc.

 

Qualifications:

Minimum –

  • One to three years of experience working for a public health agency or nonprofit

Preferred –

  • Bachelor’s or master’s degree in public health, community and environmental planning, or other related field
  • Three to five years of experience working for a public health agency or nonprofit
