The incumbent in this position is expected to work an average of 20 hours per week.


The person in this position functions in a role that provides direct health support to the Adult Primary Care clients in a clinical setting.  The incumbent will be under the supervision of a Senior Community Health Nurse Supervisor. Responsibilities include assisting nursing staff and physicians by providing varied clinical and clerical support. This position involves risk of exposure to blood borne pathogens.  Selected immunizations are a prerequisite for this position.

  1. Clinic Support Functions
    1. Welcomes clients and instructs them in preparation for their visit, reviews paperwork for completeness and accuracy, and reviews chart for laboratory tests.
    2. Prepares client for clinic visit; performs height, weight, BMI, temperature, vision and audio screening, date of last menstrual period, and enters the information into the client’s record.
    3. Assists professionals in maintaining optimum clinic flow, identifies problems and seeks solutions in concert with clinic staff. Assists with client routing in the clinic area.
    4. Prepares inventories and helps with ordering. Assists in maintaining inventory of medical supplies, checks supplies in exam rooms daily, makes a list of items needed and restocks exam rooms as necessary. Monitors patient education material supplies and replenishes supply as needed before and during clinic sessions.
    5. Assists physicians with patient care and procedures. Sets up trays and instruments for procedures, passes instruments and suture to provider. Cleans, sterilizes and store equipment after procedure.
    6. Performs electrocardiograms.
    7. Obtains patient charts from Medical Records Department as needed and returns them after clinic sessions.
    8. Maintains a clean and safe work environment.
    9. Obtains patients signature on consent forms as needed per written protocol.
    10. Supports the laboratory staff as needed in order to maintain patient flow.
    11. Performs required controls for glucometer.
  2. Clerical Duties
    1. Performs copying, collating, filing and other clerical duties as assigned.
    2. Provides telephone relief as needed.
    3. Upholds standards in record keeping and confidentiality at all times in all areas.
    4. Assists CHN with obtaining hospital records and outside referral reports.
    5. Assists in preparation of laboratory requisitions and required paperwork for clinic visit; assists with patient appointment reminder calls. 
    6. Assists with obtaining medication authorizations.
    7. Fax and document medication renewal requests.
    8. Enters HIV referrals and medications into HMS.
  3. Other duties as assigned
    1. Participates in in-service training, QI and agency meetings as assigned.
    2. Performs other related duties as assigned.
