Your Specific Responsibilities:
- Through the Communicable Disease/Tuberculosis Program this position will be responsible for the investigation, surveillance and reporting of Reportable Diseases and conditions of public health importance, case management/follow-up of cases, suspects, referrals and other direct nursing service in a variety of settings, e.g., home, clinic, schools, daycare centers, nursing homes, healthcare facilities, etc., utilizing the current policies and procedures of the Department of Health in Sarasota County (DOH-Sarasota), the Florida Department of Health (DOH) and the Centers for Disease Control and Prevention (CDC). This position reports to the Operations Manager C-SES of the FDOH- Sarasota County.
- Conducts case management/contact investigations for Tuberculosis (TB) suspects/cases, contacts and Latent TB Infection (LTBI) clients effectively following and utilizing Florida Department of Health in Sarasota County (DOH-Sarasota), DOH and CDC policies and procedures.
- Participates in and/or conducts clinic services, including but not limited to:
- Tuberculosis nursing/MD clinics
- Contact screening clinics
- Runs TB reports monthly to monitor program activities.
- Assists with maintaining and updating the DOH-Sarasota TB Program Procedure Manual.
- Provides technical assistance to DOH-Sarasota supervisors, staff and the community regarding the TB and the TB Program.
- Maintains accurate and complete medical records. The employer, Department of Health, County Health Department is the records owner of the medical records as defined by Section 455.667 F.S.
- Serves as the subject matter expert for DOH Sarasota and community partners related to TB questions and concerns
- Educates client and/or family members regarding reportable diseases, disease process, and transmission and control measures
- Coordinates and conducts TB clinic services
- Is responsible for maintaining and updating the DOH-Sarasota TB Program Procedure Manual
- Assists with maintaining and updating the DOH-Sarasota Reportable Disease Procedure Manual
- Conducts investigation, surveillance and reporting of communicable diseases, provides and other direct nursing services in a variety of settings, e.g., home, clinic, schools, day care centers, nursing homes, etc.
- Performs nursing appraisals.
- Educates client and/or family members regarding, health, disease process and transmission.
- Refers and coordinates services to ensure continuity of care.
- Acts as contact/resource person for the DOH-Sarasota.
- Provides educational/in-service presentations to the DOH-Sarasota staff and the community.
- Provides Sexually Transmitted Diseases (STD) Field Delivered Therapy services for STD clients following established CDC Florida Department of Health and DOH-Sarasota guidelines.
- Provides nursing services to HIV positive inmates in the Sarasota County Jail
- Works with DOH Sarasota physician to provide medical care to HIV positive inmates
- Provides education and counseling to cases
- Documents physician/nursing notes in Health Management System (HMS)
- Runs monthly HMS reports for invoice purposes.
- Miscellaneous Activities
- Attends and participates in DIS team meetings, ECDC staff meetings, DOH-Sarasota staff meetings and staff development activities.
- Maintains client confidentiality in a manner consistent with Chapter 384.29, F.S. and DOH policy.
- Other duties as assigned

Qualifications:
Minimum –
- Must hold an active Registered Nurse License in the State of Florida (F.S. Reference: 464) or Multistate RN License from another Compact State
- Three years of experience as a TB case manager
Preferred –
- Bachelor of Science degree in Nursing
