The Senior Community Health Nurse functions at an advanced level within the Specialty Services Clinic at the Sarasota County Health Department (SCHD). Primary responsibilities include providing comprehensive healthcare services in Maternity, HIV, Sexually Transmitted Diseases (STD), Family Planning and Hepatitis Clinics, as well as conducting HIV counseling, testing, and health screenings.
Your Specific Responsibilities:
- Clinical Care & Direct Patient Services
- Provides public health nursing services in Ringling and North Port Specialty Clinics in accordance with established standards and policies.
- Participates in Maternity, HIV, STD, Family Planning, and Hepatitis Clinics, ensuring timely patient care and follow-ups.
- Conducts comprehensive nursing assessments during clinic visits, applying evidence-based practices.
- Ensures continuity of care through effective case management for patients with chronic or acute conditions.
- Collaborates closely with Ryan White case managers to optimize HIV care services.
- Assists uninsured patients in accessing Hepatitis C program services and Patient Assistance Program (PAP) applications for medication support.
- Maintains comprehensive and accurate client records, ensuring compliance with documentation standards.
- Assists healthcare providers during examinations and medical procedures.
- Provides health education and counseling to clients and their families, promoting disease prevention and wellness.
- Performs HIV pre- and post-test counseling following agency and DOH guidelines.
- Conducts laboratory specimen collection (urine, capillary, and venous blood), ensuring proper handling and processing.
- Assists Advanced Practice Registered Nurses (APRNs) and Doctors in delivering specialty clinical care.
- Patient Coordination & Continuity of Care
- Facilitates internal and external referrals, ensuring clients receive the necessary follow-up services.
- Participates in service coordination to optimize testing, treatment, and ongoing patient management.
- Clinical Support & Compliance
- Participates in Quality Assurance/Improvement activities and medical record audits.
- Assists in training and orienting new staff and students, ensuring knowledge transfer and adherence to best practices.
- Provides support in other clinics or units as needed.
- Assists in insurance claim resolution by providing necessary information to avoid denials and ensure timely payment processing.
- Additional Responsibilities
- Engages in ongoing professional development, including in-service training and continuing education.
- Attends annual information security awareness training and adheres to department policies.
- Maintains active nursing licensure and meets all continuing education requirements.
Required Knowledge, Skills, and Abilities:
- Advanced knowledge of nursing practice
- Knowledge of OB, STDs, HIV, Hepatitis C, and Family Planning

Qualifications:
Minimum –
- Current Florida DOH employee
- Current, active Florida Registered Nursing license or a multistate license from another Compact State
- One to three years of clinical experience working as a registered nurse
- Experience in Women’s Health/OB
Preferred –
- One year of experience with electronic health records
- One to three years of experience in phlebotomy
- English/Spanish bilingual
