Your Specific Responsibilities:

 

  1. Case Management Services

Provides overall coordination of the FBCC in cooperation with the Executive Community Health Nursing Director

Admits clients into the program; obtains screening/diagnostic results from other agencies, and notifies clients of screening/diagnostic results

Follows-up on abnormal results to ensure client receives care as ordered by attending physician

Refers client to community resources as needed

Documents in Health Management System (HMS), Volusia system other systems as needed

Collects and signs off on bills/invoices from providers

Collaborates with professional and support staff regarding problem areas

Assures required program reports are prepared and submitted per program guidelines

Assures proper budget management and adherence to all programmatic guidelines

Assures timely submission of grant required reports for the FBCC

Ensures all Patient Reporting Forms (PRF’s) are submitted to program office per program requirements.

Ensures proper documentation of all case management services for FBCC

 

  1. Outreach Activities

Performs site visits to FBCC providers as needed to educate, evaluate client records and billing systems and to provide client educational materials

Performs outreach efforts which may also include meeting with local women’s groups, church groups, private organizations, and screening promotions

Regularly participates in health fairs to promote the program about breast and cervical cancer screening

Documents program activities and outreach efforts and reports to supervisor monthly

Establishes Memorandum of Agreement/Understanding with  Mobile Mammography to increase number of women that are enrolled and screened through the FBCCEDP

Establishes Memorandum of Agreement/Understanding with Sarasota Memorial Hospital to provide a joint, yearly   community outreach event to increase number of women that are enrolled and screened through the FBCC

Plans future goals to incorporate additional community partners to increase mammogram/PAP referrals

Plans future goals to increase planned outreach mobile mammography events.

Manages, coordinates, and staffs the increased number of outreach mobile mammography events a minimum of 4 times per year.

 

  1. Works cooperatively with other team members to maintain quality of services delivered in all tasks associated with position. Keeps supervisor informed of any program developments or occurrences outside the realm of standard operating procedures.

 

  1. Other Related Duties

Participates in team meetings, general staff meetings, program conferences and professional trainings

Completes all mandatory trainings as outlined in Florida Department of Health (FDOH) policy

Completes Employee Activity Reports (EARS) in HMS and submits time reports in People First within weekly in accordance with FDOH policy

 

 

Required Knowledge, Skills, and Abilities:

  • Knowledge of nursing principles, practices, and techniques
  • Knowledge of human anatomy and physiology
  • Ability to accurately interpret FBCC guidelines and regulations for service delivery and disseminate the information to coworkers and providers
  • Ability to accurately complete complex paperwork associated with FBCC
  • Ability to prepare clear and concise reports
  • Ability to effectively educate providers about the program guidelines, eligibility requirements, billing, etc.
  • Ability to effectively educate the public about Clinical Breast Exams (CBE), monthly self-checks, mammograms & Papanicolaou (PAP), and to inform eligible women on program requirements
  • Ability to manage and track budget
  • Ability to communicate effectively and courteously in a culturally sensitive manner
  • Ability to maintain effective working relationships with others
  • Ability to solve problems creatively
  • Ability to prioritize and organize tasks for maximum efficiency
  • Must possess basic computer skills
  • Must be able to travel throughout the eight-county region             

 

Qualifications:

Minimum

  • Associate degree in Nursing
  • Licensure as a Registered Nurse in accordance with F.S. 464
  • Valid Florida driver’s license
  • One year of work experience as a Registered Nurse

 

 Preferred

  • Women’s Health experience
