SENIOR COMMUNITY HEALTH NURSE

Your Specific Responsibilities:
- Offers float support in all Clinical Areas within Community and Clinical Health Space
- Provides direct nursing care in the Florida Breast and Cervical Cancer Early Detection Program (FBCC) and Maternity/OB clinics as assigned based on daily staffing needs
- Provides direct nursing care in the STD, HIV/Ryan White, and Same Day Testing clinics as assigned based on daily staffing needs
- Conducts sexual and risk history, symptom assessment, and specimen collection per CDC STI Treatment Guidelines and Department protocols
- Administers treatment and expedited partner therapy where permitted, per standing orders and provider direction
- Performs rapid HIV, syphilis, and hepatitis C point-of-care testing with pre-test and post-test counseling per Florida Department of Health counseling and testing standards
- Delivers preliminary positive results and coordinates same-day confirmatory testing and rapid linkage to care, including rapid antiretroviral initiation when available
- Facilitates same-day PrEP and nPEP assessment, baseline labs, and provider handoff
- Supports HIV primary care visits, including vital signs, medication reconciliation, adherence assessment, and laboratory collection (viral load, CD4, and resistance testing as ordered)
- Reinforces antiretroviral adherence education and assists with Ryan White eligibility and recertification documentation as assigned
- Performs CLIA-waived testing quality control and maintains testing logs per laboratory policy
- Follows up on abnormal results to ensure clients receive diagnostic services and treatment as ordered by the attending provider
- Refers eligible FBCC clients to the Mary Brogan Breast and Cervical Cancer Medicaid pathway for treatment
- Coordinates syphilis and congenital syphilis follow-up with Disease Intervention Specialists (DIS), including pregnancy status screening
- Monitors missed appointments and laboratory results and initiates re-engagement outreach for clients out of care or not virally suppressed
- Tracks high-risk prenatal referrals and coordinates with delivering hospitals, case managers, and community partners
- Refers clients to community resources as needed
- Completes standardized handoff of all pending follow-up before rotating to another clinic assignment
- Documents all services accurately and timely in the Health Management System (HMS), Florida SHOTS, and program data systems
- Completes reportable disease documentation and reporting per Chapter 64D-3, Florida Administrative Code
- Ensures program forms and data required for FBCC, Ryan White, STD, Title X, and maternity reporting are complete and submitted per program requirements
- Maintains confidentiality of client health information per HIPAA, Section 381.004, Florida Statutes (HIV), Section 384.29, Florida Statutes (STD), and Department information security policy
- Reports suspected abuse, neglect, or exploitation per Chapters 39 and 415, Florida Statutes
- Participates in chart audits, program site reviews, and corrective action plans
Required Knowledge, Skills, and Abilities:
- Ability to be flexible regarding work schedules and locations
- Knowledge of nursing principles, practices, and techniques
- Knowledge of human anatomy and physiology
- Ability to accurately interpret Family Planning, OB, 540B, Ryan White, and FBCC guidelines and regulations for service delivery and disseminate the information to coworkers and providers
- Ability to accurately complete complex paperwork associated with Community and Clinical Health Space
- Ability to prepare clear and concise reports
- Ability to effectively educate providers about the program guidelines, eligibility requirements, billing, etc.
- Ability to effectively educate the public about Clinical Breast Exams (CBE), monthly self-checks, mammograms & Papanicolaou (PAP), and to inform eligible women on program requirements
- Ability to manage and track budget
- Ability to communicate effectively and courteously in a culturally sensitive manner
- Ability to maintain effective working relationships with others
- Ability to solve problems creatively
- Ability to prioritize and organize tasks for maximum efficiency
- Basic computer skills
Qualifications:
Minimum –
- Licensure as a Registered Nurse in accordance with Florida Statute 464
- Valid driver’s license
- Knowledge of compliance regarding state-sponsored programs
Preferred –
- Bachelor’s degree of higher
- History and background in one or more of the following clinical programs: OB, Family Planning, Female Health, FBCCP, STD/HIV care
