OPS SENIOR LICENSED PRACTICAL NURSE (ADMISSIONS)

MINIMUM QUALIFICATIONS:

  • Licensure as a Practical Nurse in accordance with F.S. 464 or eligible to practice nursing in accordance with F.A.C 210-822 or 8.27; or a 2-year associates (AA) degree.
  • One (1) year of experience in providing practical nursing services, case management, marketing, or sales.
  • Excellent communication and interpersonal skills.
  • Successful completion of a Level 2 background screening and Drug Test.

 

PREFERRED QUALIFICATIONS:

  • Proficient computer skills, including Microsoft Office Suite and Web-based applications.
  • Marketing experience / skills.

 

POSITION DESCRIPTION:

  • Incumbent will work under the supervision of the Administrator and is responsible for the resident admission process and documentation of all components of that process for the State Veterans’ Nursing Home.
  • Interview and counsel potential residents for possible placement within the facility.  Greet visitors and conduct tours of the facility for prospective residents and their families.  Coordinate compilation of required pre-admission and admission documentation.  Prepare resident applications, schedule admission, coordinate room assignments and establish a medical record.
  • Coordinate with the facility Business Manager and Veterans’ Claims Examiner regarding veterans’ maximum benefit entitlement and benefits being received.  Interact with Medicaid, Medicare and Social Security Administration, verify Medicare eligibility and assist the resident and family in initiating the Medicaid application.  Interact with the US Department of Veterans’ Affairs Regional Office to prepare and submit required documentation.
  • Receive and respond to related incoming mail.  Maintain documentation tracking the status of applications received and contacts with potential residents.
  • Initiate and conduct marketing of the nursing home to hospital discharge planners, private sector nursing homes and other agencies.  Establish and maintain public relations with and contact information of area hospital discharge planners, Social Services managers and nurses.  Maintain contact with the Veteran’s Service Officers at the VA Medical Centers, County Veterans Service Officers, local, federal and other state agencies to assist veterans.  Maintain close coordination with the Bureau Chief of Field Services.  Visit hospital and other referral source meetings as part of the facility marketing plan.
  • Present potential resident application to the facility Admissions Committee and coordinate the admission process with facility department heads.
  • Reviews medical information as part of the admission evaluation process.  Coordinate resident health issues and special needs with appropriate department heads to ensure resident needs are met.  Complete nursing intake forms.  Maintain computer office files and records to include correspondence, documents, forms and reports.  Maintain preadmission and admission checklist information for each resident in the facility.  Compile and analyze admission data.  Prepare related memos, correspondence and reports as requested by the NHA. 
  • Maintain an adequate library of current informational regulatory directories for use as reference material, facility marketing information and tools.  Maintain and update current Veteran/applicant benefit information. 
  • Perform other relate duties as required.  Attends conferences, meetings and seminars as needed.
