Human Services Program Specialist – OPEN COMPETITIVE

 

Your Specific Responsibilities:

Access Coordination

  • Serve as patient advocate by coordinating access to available services under the PCPH program.  Works with Pinellas County Health and Human Services to link clients with other services as needed, such as rent assistance, job training, disability claims, bus passes/transportation, applications for food stamps/Medicaid and emergency housing.  Researches and maintains community resource information/handouts provided to PCHP clients.  Mails community resources/events to PCHP clients as needed.
  • Ensures newly enrolled clients receive appointments within the established timeframes
  • Coordinates patient specialty care referrals, which includes mailing referral letters and answering questions regarding the status of specialty care referrals.  Assists clients with scheduling specialty care appointments and follows-up to ensure client was seen by specialists.  Obtains medical records from specialist as needed.
  • Processes PCHP enrollment and disenrollment reports to ensure new clients get established with the medical home and disenrolled clients are provided with the necessary information to re-enroll if needed.

 

Outreach

  • Ensures client engagement with the medical home by working with contract staff and community partners to obtain current contact information for clients.

 

Clinic Processes

  • Serves as back-up to Primary Care Senior Clerk as needed.
  • Schedules and reschedules client appointments, places appointment reminder calls.  Contacts clients that miss appointments (no shows).

 

Reporting

  • Perform weekly and monthly record keeping and reporting of data and statistics related to those served
  • Assist in the preparation of reports as needed

 

Miscellaneous

  • Attends and participates in staff meetings, in-service training, and other professional and staff development activities
  • Performs other related duties and responsibilities as assigned

Required Knowledge, Skills, and Abilities:

  • Knowledge and utilization of multi-line telephone system
  • Skills in use of personal computer and EMR, software required to verify client status/eligibility
  • Knowledge of medical terminology.
  • Ability to effectively communicate verbally and in writing, utilizing approved DOH abbreviations when documenting in the EMR.
  • Knowledge of methods of compiling, organizing, and analyzing data.
  • Ability to plan, organize, and coordinate work assignments
  • Ability to maintain accurate and appropriate medical documentation in electronic medical record.
  • Have a Florida driver’s license and have dependable transportation

 

Qualifications:

Minimum – 

  • 2 years’ experience using a computer
  • Experience working in a healthcare setting
  • Strong organizational skills with the ability to multitask
  • Excellent verbal and writing communication skills
  • Ability to work independently
  • Have a valid Florida driver’s license with access to dependable transportation

 

Preferred -

  • Experience working in Public Health
  • Experience working with community and co-located partners
  • Experience working with an electronic health record
