Your Specific Responsibilities:
This is a Health Services Representative position that provides support for the multidisciplinary Epidemiology and Disease Control team in Martin County, under the supervision of the Health Services Supervisor-SES. Performs field and in-clinic services for Tuberculosis control and prevention, working with active and latent TB clients. Participates in HIV Prevention and testing initiatives and assists in linking patients to care. Provides programmatic support and surge capacity for the Epidemiology Program.
TB. Provides Directly Observed Therapy (DOT) and Directly Observed Preventive Therapy (DOPT) in clinic and in the field, or by pre-approved video application. Enroll clients in the Video application and teach them how to operate it. Observes patients for adverse reaction to medications. Documents all activities in the DOT logs and Health Management System (HMS) and uploads all documents in HMS. Determines compliance with treatment plan and reports any problems to the Nurse Case Manager (NCM) or supervisor. Orders medication refills as needed or as requested by NCM and Supervisor.
Assists in TB investigations and contact tracing. Provides patient and family with education on Tuberculosis and TB Program processes of care. Draws blood and completes TB testing for community members as required by their school or worksite, under supervision of NCM. Enters radiology orders in HMS and provide patient with order and DOH authorization. Collects discs with imaging from hospitals or clinics. Uploads images in Infinitt.
Provides transportation for patients in state vehicle as needed as it relates to TB and travels to patients’ house or other agreed-upon locations. Collects and transports specimens such as sputa and blood. Enters orders in HMS, labels specimens appropriately and processes specimens for shipment. Monitors lab results and consults with NCM as needed.
Assists NCM in meeting all programmatic and case management requirements and completing RVCTs. Works with clerical team to ensure patients are properly registered and scheduled. Ensures proper documentation on encounter forms. Answers phone and returns calls related to TB. Works on EDNs and IJNs as requested by NCM or Supervisor. Assists in requesting and delivering incentives and/or enablers.
HIV. Engages in HIV Prevention activities. Participates in community education, outreach events, HIV/STD testing and counseling in the community and in clinic setting. Shares information about HIV and STD testing, PrEP, treatment, linkage to care. Sees walk-in patients. Keeps clients’ information anonymous and confidential following the DOH Counseling and Testing protocols. Enters rapid test lab results in HMS and services in a timely manner. Enters HIV and STD orders in HMS. Links individuals testing positive into the care system in collaboration with the Linkage to Care Program and utilizing referral process.
Serves as back-up to ADAP program, assisting with: scheduling appointments, client enrollment and re-enrollment, uploading files in Provide Enterprise, verification of timely medication pick-up, and other program requirements.
Epidemiology. Provides programmatic support to the Epidemiology Program. Assists the Epidemiology program with case investigations, including case interviews, documentation in Merlin, contact tracing, specimen pick-up and shipment, answering phones, clerical support, and overall surge capacity.
Miscellaneous activities. Obtains Packaging and Shipping certification and 500/501 Certification and keeps both certifications current according to renewal guidelines. Attends and participates in general staff meetings and staff development, such as team/staff meetings and in-service/ continuing education; performs related duties as necessary including public health services during a disaster (shelter duty); maintains timely submission of EARs and timesheets. Other duties as assigned.
Required Knowledge, Skills, and Abilities:
Knowledge of: medical terminology pertaining to tuberculosis and HIV; communicable disease control methods, activities, and protocols; public health policies and public health laws to include but not limited to information security and HIPAA; community resources; case management processes.
Skills in: phlebotomy; time management and prioritization of tasks; interviewing cases/patients to gather health information and elicit contacts; conducting epidemiological investigative case follow up; DOH disease surveillance systems; computer software programs used to facilitate job duties such as Microsoft Office Suite; use of medical equipment and instruments used in providing support care.
Ability to: read and correctly interpret medical records; work independently using critical thinking and logic; pay attention to detail and perform investigations in a timely fashion; use problem-solving techniques; communicate effectively verbally and in writing with clients, co-workers, other professionals and in group; establish and maintain effective working relationships; persuade others; understand and apply epidemiologic investigation techniques and use epidemiologic resources (Red Book, CCDM, GSI) to perform investigations; access and extract information from multiple databases; recommend appropriate disease control measures; document work products.
Minimum Qualifications:
High School Diploma/GED
Willing to work before, during and after an emergency/disaster at a special needs shelter
Willing to work before and after normal business hours to accommodate patients’ needs and occasionally work on outreaches on weekend and after-hours
Willing to work with patients with infectious diseases such as TB
Willing to undergo FIT testing
Have a valid Florida Driver License
Experienced in phlebotomy and able to draw blood independently
Preferred Qualifications:
1 year working with clients in public health or healthcare
