Your Specific Responsibilities:

 

Duties and Responsibilities

This is a professional Registered Nurse (RN) position with the Florida Department of Health- Martin County in the School Health Program under the direct supervision of the Senior Community Health Nursing Supervisor. This is a ten-month career service position and works only when school is in session; utilizes annual leave or leave without pay plus personal checks to maintain state benefits when not in a working status. Maintains quality of services delivered in all tasks associated with position as evidenced in periodic evaluation/monitoring performance of specific competencies. The incumbent in this position must be self- motivated and work with limited supervision.

 

CASE MANAGEMENT

  • Acts as patient advocate and nurse case manager for students with acute and chronic medical conditions
  • Coordinates medical services between parents/guardians and specialty medical offices
  • Completes referrals for identified acute and chronic medical conditions in writing, telephone calls and home visits as indicated
  • Creates and refers students to Children’s Emergency Resources (CER).
  • Creates CER medical voucher to obtain medical/dental services and medical/dental equipment
  • Creates and refers students to the Capen Fund for Kids program
  • Follows up to ensure satisfactory outcomes are achieved from referrals and case management
  • Obtains medical equipment and supplies, brings to student’s school or home when indicated
  • Provides parents/guardians with education on disease processes, treatments- medications and medical equipment use
  • Documents health information accurately and thoroughly in a timely manner using FOCUS and HMS
  • Attends IEP and 504 meetings as requested by the School District of Martin County staff
  • Documents case management hours in HMS using service code 9010, maintains case management log
  • Serves as back-up nurse case manager when another DOH school health nurse is on leave
  • Prepares Children’s Services Council (CSC) Martin County Mid-Year and End of Year training log
  • Uses FOCUS to review student medical records including IHCP/EAP/135 forms, demographics, etc.
  • Maintains open communication with the School District of Martin County staff
  • Multidisciplinary collaboration between school district, hospitals, medical offices, and parents/guardians
  • Prepare end of year case management reports for summer transition and preparation for following school year
  • Communicate with parents/guardians and medical providers on summer leave to ensure case management continuity of care

 

HEALTH SCREENINGS

  • Schedules, coordinates, documents, and completes mandated health screenings and any health screening referrals from the School District of Martin County
  • Provide assigned schools with Mandated Health Screening Opt Out Forms
  • Pick up signed opt out forms from schools and provide to School Health staff for processing
  • Ensure opt out form is complete, if not contact parent/guardian to explain form, send another form to parent/guardian
  • Creates referrals to appropriate medical providers due to failed health screenings
  • Provide case management for failed health screenings
  • Communicates with parents/guardians on failed health screening results and referrals
  • Ability to lift or carry equipment weighing 20 pounds or more.
  • Uses proper body mechanics to provide health screenings to students. Nurse will have to bend, stoop, or stretch to accommodate the correct screening technique.
  • Trains nursing students or Medical Reserve Corps (MRC) volunteers on health screening forms, use of equipment, how to properly screen students, referral, and case management process

 

IMMUNIZATIONS

  • Utilizes FL Shots (Florida vaccine registry) to obtain, transcribe out of country or state records, and input immunizations into electronic vaccine database
  • Creates DH 680 in FL Shots and provides to designated District RN
  • Serves as subject matter expert for immunization schedule and record transcription
  • Ensures students in nurse’s assigned schools are in compliance with current ACIP/CDC recommendations and county/state requirements.
  • Administers immunizations during scheduled clinics
  • Follows up with the School District of Martin County personnel and/or parents/guardian to assure immunization compliance
  • Works with Nurse Program Specialist for Immunizations to schedule vaccine appointments in immunization clinic
  • Completes annual Vaccine for Children (VFC) training on vaccines and vaccine storage and handling

 

TRAININGS/MEETING

  • Completes required trainings
  • Attends meetings, conferences, workshops, etc. to maintain level of nursing competency and licensure

Attends monthly all staff and School Health program meeting

 

OTHER DUTIES

Perform other related work when requested.

 

Required Knowledge, Skills, and Abilities:

Knowledge: Nursing principles, practices, and techniques; human anatomy and physiology; aseptic technique and universal precautions; adhering to state statutes, rules/regulations, and practices.

Skills: Possess basic computer software skills in Microsoft software programs including Word, Excel, PowerPoint, and Outlook; uses office and necessary systems/software (FL Shots, FOCUS) related to position. Skill to use equipment such as audiometer, vision spot screeners and any other necessary equipment related to position. Administer immunizations in clinic.  

Ability: Independently manage multiple work assignments per applicable State of Florida and DOH rules. Communicate effectively, courteously, and professionally at all times (both verbal and written); maintain effective working relationships with internal and external customers; problem solving and creative thinking towards solutions; prioritize and organize tasks for maximum efficiency; relate with clients and the public in a culturally sensitive, respectful manner; and review data for accuracy and completion. Accept constructive criticism and respond and implement suggestions for improvement. Transport, lift, and use program equipment up to 25 pounds and must be able to move around freely

 

Qualifications:

Minimum-

  • Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 or 8.27
  • Have 2 years of current Registered Professional Nursing experience                                                                     
  • Have a valid driver’s license                                                                                                                                 
  • Must be able to work before, or beyond regular work hours or days in the event of an emergency 
  • Currently certified in CPR/AED & First Aid
  • Have experience working with children                                                                                         

 

Special Notes About This Position:

The duties include but are not limited to Nurse Case Management of students with acute or chronic conditions, working with community partners, providers, and resources, including home visits, providing parental education, complete referrals and outcomes, and accurately document in electronic systems. Participate in all mandated Health Screenings.  Participate in back-to-school immunizations, complete immunization documentation, and record keeping. Attend mandatory trainings, conferences, workshops to maintain level of nursing competency and licensure. Attend all staff and School Health program meetings, and completes other duties as assigned, including emergency duty.

 

Preferred experience but not required for this position is DOH School Health experience, Nurse Case Management experience, persons who speak other languages than English such as Spanish.  Persons who have experience working with Microsoft word, excel, power point and outlook.  Persons with experience working with HMS, FOCUS, FLSHOTS, or other school-based systems, persons with clerical skills, and persons with patient and/or family education experience are all helpful for this position.
